摘要:
目的 探究巨幼红细胞性贫血(megaloblastic anemia, MBA) 患者血浆维生素B9(VB9) 和维生素B12(VB12) 水平对p53 表达的影响。方法 选择自2015 年3 月~ 2018 年5 月68 例MBA患者为病例组,另选取同期68 例健康者为对照组。分别比较VB9 和VB12 的分布情况、比较p53 在不同程度VB9 和VB12 分布中的表达水平、比较两组间同型半胱氨酸(Hcy) 的含量和分布情况和分析低VB9 和VB12 的MBA 患者同型半胱氨酸的含量与p53 表达的相关性。结果 病例组VB9 和VB12 水平分别为5.28±0.36ng/ml 及144.51±28.17pg/ml,显著低于对照组(5.73±0.45ng/ml,176.33±31.54pg/ml), 差异有统计学意义(t=-6.44,-6.20,均P<0.001)。在VB9 和VB12 含量均偏低时,病例组患者的分布频数显著高于对照组(t=4.847,P < 0.05)。在VB9 和VB12 含量均偏低时,病例组细胞p53 表达中度和重度染色的比例较高,与其余两组病例组的差异具有统计学意义(t=7.551,P < 0.05)。病例组的同型半胱氨酸含量为43.2μmol/L,且同型半胱氨酸患者分布比例为80%,显著高于对照组(t=4.226,P < 0.05)。在VB9 和VB12 含量均偏低时患者中同型半胱氨酸含量与p53 表达呈正相关性(Y=0.655X+12.42,r=0.661)。结论 p53 的高表达可作为低VB9 和VB12 水平诱导MBA产生的标志物,为MBA 的诊断提供有效参考。
