摘要:
目的 探讨血清中胎球蛋白(Fetuin B)水平与 2 型糖尿病(T2DM)患者下肢动脉粥样硬化性病变(LEAD)的关系。方法 选取 2018 年 7 月~ 2019 年 3 月,中山大学附属中山医院内分泌科治疗的 122 例 T2MD 患者作为研究对象,分为 T2DM 组和 LEAD 组,其中 T2DM 组患者 41 例,LEAD 组 81 例。选取同时期 45 例健康体检者作为对照组。分析 LEAD 患者血清 Fetuin B,血压、肝肾功能、血脂、血糖、尿酸和 Hcy 水平变化情况。结果 对照组、T2DM组和 LEAD 组血清中 Fetuin B 水平呈逐渐升高趋势,血清中 Fetuin B 水平分别是 9.384±5.014,55.700±37.902 和58.752±69.377 ng/ml,LEAD 组与对照组和 T2DM 组比较,差异有统计学意义(F=13.962,P<0.05);二元 logistic 回归分析,Fetuin B 和 TG 是 LEAD 患者的独立危险因素。ROC 曲线结果,血清中 Fetuin B 在 19.639 ng/ml 时,对 LEAD患者具有较好的诊断价值(AUC 0.835,灵敏度0.700,特异度0.977)。TG在1.375mmol/L时,具有较好的诊断价值(AUC0.773,灵敏度 0.625,特异度 0.841)。结论 血清中 Fetuin B 升高可能会使 T2DM 患者增加双下肢动脉粥样硬化的风险,监测血清中 Fetuin B 水平的变化情况,有助于早发现 T2DM 病患者双下肢动脉粥样硬化病变的发生,为临床早诊断早治疗提供一定的依据。
