摘要:
目的 探讨缺血性脑卒中患者血清中长链非编码RNA(long-chain non-coding RNA,LncRNA)母系表达基因8(maternal expressed gene 8,MEG8)的表达及其临床价值。方法 选取2021 年1 月~ 2022 年4 月在海南省老年病医院接受治疗的80 例缺血性脑卒中患者为观察组,及同期85 例健康体检志愿者为对照组。参照美国国立卫生研究院卒中量表(NIHSS)评分将患者分为轻度患者组(n=25,NIHSS<5 分)、中度患者组(n=29,5 ≤ NIHSS ≤ 20 分)和重度患者组(n=26,NIHSS>20 分)。采用实时荧光定量PCR(qRT-PCR)法对受试者血清中LncRNA MEG8 的水平进行测定并对不同程度组患者血清LncRNA MEG8 水平进行比较。患者血清中LncRNA MEG8 与C- 反应蛋白(CRP)水平的相关性进行Pearson 分析;Spearman 法对患者LncRNA MEG8 与NIHSS 评分的关系进行分析;Logistic 分析影响缺血性脑卒中发生的因素;受试者工作特征(ROC)曲线分析LncRNA MEG8 水平诊断缺血性脑卒中的价值。结果 与对照组相比,观察组血清中LncRNA MEG8 显著升高(1.55±0.31 vs 1.01±0.15),差异有统计学意义(t=14.373,P=0.000);与轻度患者组(1.26±0.18)相比,中度与重度患者组LncRNA MEG8 表达水平随着病情的加重依次显著升高(1.54±0.27,1.84±0.48),差异有统计学意义(F=19.262,P=0.000);缺血性脑卒中患者血清中LncRNA MEG8与CRP 水平、NIHSS 评分呈正相关(r=0.412,0.488;P=0.000 5,0.000 3);高血压(OR=3.314,95%CI 1.045 ~ 10.513)、高脂血症(OR=4.340,95%CI 1.008 ~ 18.692)、糖尿病(OR=7.891,95%CI 1.024 ~ 60.828),高LncRNA MEG8(OR=7.021,95%CI 4.316 ~ 37.440)和高CRP(OR=2.415,95%CI 1.207 ~ 4.833)是发生缺血性脑卒中的危险因素(P=0.042,0.048,0.047,0.022,0.013);LncRNA MEG8 水平诊断缺血性脑卒中的ROC 曲线下面积为0.923(95%CI: 0.880 ~ 0.966),截断值为1.14,敏感度和特异度分别为81.3%,94.1%。结论 缺血性脑卒中患者LncRNA MEG8 表达升高,与患者病情严重程度相关,可较好地诊断缺血性脑卒中。
Abstract:
Objective To investigate the expression of maternal expression gene 8 (MEG8) of long-chain non-coding RNA (LncRNA) in the serum of patients with ischemic stroke and its clinical significance. Methods 80 patients with ischemic stroke who were treated in Geriatric Hospital of Hainan from January 2021 to April 2022 were regarded as the observation group, and 85 healthy volunteers in the same period were included as the control group. The severity of all patients was evaluated according to the National Institutes of Health Stroke Scale (NIHSS), which was grouped into mild patients (n=25,NIHSS<5 score), moderate patients (n=29,5 ≤ NIHSS ≤ 20 score) and severe patients (n=26,NIHSS>20 score). The relative expression level of LncRNA MEG8 in serum of all subjects was detected by real-time fluorescent quantitative PCR (qRT-PCR), and compared between patients in different degrees group. The correlation between LncRNA MEG8 and C-reactive protein (CRP) levels in serum of patients with ischemic stroke was analyzed by Pearson. Spearman method was used to analyze the correlation between LncRNA MEG8 and NIHSS score in patients with ischemic stroke. Logistic regression was performed to analyze the factors influencing the occurrence of ischemic stroke, the diagnostic value of the expression level of LncRNA MEG8 in ischemic stroke was analyzed by ROC curve. Results The expression level of serum LncRNA MEG8 in control group and observation group were 1.05±0.15 and 1.55±0.31, respecitvely. Compared with the control group, the level of LncRNA MEG8 in serum of the observation group was obviously higher, the difference was statistically significant (t=14.373, P=0.000). Compared with mild patients group (1.26±0.18), the level of LncRNA MEG8 expression in moderate and severe patients group increased significantly with the aggravation of the disease (1.54 ± 0.27, 1.84 ± 0.48), the difference was statistically significant (F=19.262,P=0.000). The serum LncRNA MEG8 in patients with ischemic stroke was positively correlated with CRP level and NIHSS score (r=0.412, 0.488, P=0.000 5, 0.000 3).Logistic regression analysis showed that hypertension (OR=7.891, 95%CI:1.024 ~ 60.828), hyperlipidemia (OR=4.340, 95%CI:1.008 ~ 18.692), diabetes (OR=7.891, 95%CI:1.024 ~ 60.828), LncRNA MEG8 (OR=7.021, 95%CI:4.316 ~ 37.440), CRP (OR=2.415, 95%CI:1.207 ~ 4.833) were risk factors for ischemic stroke (P=0.042, 0.048, 0.047, 0.022, 0.013). The area under the ROC curve (AUC) of ischemic stroke diagnosed by the level of LncRNA MEG8 was 0.923 (95%CI:0.880 ~ 0.966), and the cut-off value was 1.14, the sensitivity and specificity were 81.3% and 94.1%, respectively. Conclusion The expression of LncRNA MEG8 was increased in patients with ischemic stroke, which is related to the severity of the patient's condition and can better diagnose ischemic stroke.
