摘要:
目的 探究IgA 肾病患者血清微小核糖核酸(micro RNA,miR)-346 及分泌型卷曲相关蛋白4(secretedfrizzled-related protein 4, SFRP-4)水平表达与病情严重程度及预后的关系。方法 选取2018 年3 月~ 2020 年1 月在成都市第六人民医院接受治疗的102 例IgA 肾病患者为研究对象,根据肾间质纤维化(renal interstitial fibrosis,RIF)程度将IgA 肾病患者分为轻度组(n=35)、中度组(n=43)和重度组(n=24),根据随访结果将IgA 肾病患者分为预后良好组(n=70)和预后不良组(n=32),另选取同期健康体检者50 例为对照组。比较患者一般资料及血清miR-346 和SFRP-4 水平;Spearman 相关性分析IgA 肾病患者血清miR-346 和SFRP-4 水平与病情严重程度之间的关系;Logistic 回归分析IgA 肾病患者预后不良的影响因素。结果 随着IgA 肾病患者病情程度的增加,血清肌酐(serum creatinine,SCr)、血尿素氮(blood urea nitrogen,BUN)、24 h 尿蛋白定量、尿酸(uric acid,UA)和SFRP-4 水平逐渐升高,差异均有统计学意义(F=183.466,66.795,46.281,88.920,147.171,均P < 0.05);血红蛋白(hemoglobin,Hb)、清蛋白(albumin,Alb)、肾小球滤过率(estimated glomerular filtration rate,eGFR)和miR-346 水平逐渐降低,差异均有统计学意义(F=15.919,55.071,132.759,96.735,均P < 0.05);Spearman 相关性分析结果显示,IgA 肾病患者血清miR-346 与病情程度呈显著负相关(r=-0.673,P < 0.05),血清SFRP-4 与病情程度呈显著正相关(r=0.532,P< 0.05);预后不良组患者SCr(164.39±29.21 mmol/L),BUN(12.29±2.79 mmol/L),24 h 尿蛋白定量(2.80±0.79g/24h),UA(398.41±52.63 μmol/L ),SFRP-4(2.90±0.41 ng/ml)水平均高于预后良好组(121.68±26.89 mmol/L,8.85±2.47 mmol/L,2.26±0.65 g/24h,367.36±43.61 μmol/L ,2.18±0.35 ng/ml),差异具有统计学意义(t=7.184,6.264,3.836,3.123,9.128, 均P < 0.05);Hb(119.56±22.25 g/ml),Alb(28.01±4.17 g/ml),eGFR(53.53±10.14 ml/min/1.73m2),miR-346(0.43±0.10 vs)水平均低于预后良好组(132.48±24.15 g/ml,34.22±5.82 g/ml,70.41±14.21ml/min/1.73m2,0.71±0.15),差异具有统计学意义(t=2.450,5.426,6.046,9.615,均P < 0.05);Logistic 回归分析结果显示,SCr,24 h 尿蛋白定量、eGFR,miR-346 和SFRP-4 是IgA 肾病患者预后不良的独立影响因素(均P < 0.05)。结论 IgA 肾病患者血清miR-346 水平下降,SFRP-4 水平升高,其与患者病情严重程度和预后的关系密切。
Abstract:
Objective To investigate the relationship between serum micro RNA (miR) -346 and secreted frizzled related protein 4 (SFRP-4) levels and severity and prognosis in patients with IgA nephropathy. Methods 102 patients with IgA nephropathy who received treatment at the Sixth People’s Hospital of Chengdu from March 2018 to January 2020 were selected as the research subjects, according to the degree of renal interstitial fibrosis (RIF), patients with IgA nephropathy were grouped into mild group (n=35), moderate group (n=43) and severe group (n=24), according to the follow-up results, patients with IgA nephropathy were grouped into a good prognosis group (n=70) and a poor prognosis group (n=32), and 50 healthy subjects during the same period were selected as control group. General information and serum levels of miR-346 and SFRP-4 were compared. Spearman correlation analysis was applied to analyze the relationship between the serum levels of miR-346 and SFRP-4 and the severity of IgA nephropathy and Logistic regression analysis was applied to analyze the influencing factors of poor prognosis in patients with IgA nephropathy. Results As the severity of IgA nephropathy increased,the levels of serum creatinine (SCr), blood urea nitrogen (BUN),24-hour urinary protein quantitation,uric acid (UA), and SFRP-4 gradually increased,and the differences were statistically significant (F=183.466, 66.795, 46.281, 88.920, 147.171, all P < 0.05). The levels of hemoglobin (Hb),albumin (Alb),estimated glomerular filtration rate (eGFR),and miR-346 gradually decreased,and the differences were statistically significant (F=15.919,55.071,132.759,96.735,all P < 0.05). Spearman correlation analysis showed that there was a obvious negative correlation between serum miR-346 and severity of IgA nephropathy (r=-0.673, P<0.05), and there was a obvious positive correlation between serum SFRP-4 level and severity of disease (r=0.532, P<0.05). The levels of SCr (164.39±29.21 mmol/L), BUN (12.29±2.79 mmol/L), 24-hour urinary protein quantitation (2.80±0.79 g /24h), UA (398.41±52.63 μmol/L), and SFRP-4 (2.90±0.41 ng/ml) in poor prognosis group were higher than those in good prognosis group(121.68±26.89 mmol/L,8.85±2.47 mmol/L,2.26±0.65 g/24h,367.36±43.61 μmol/L ,2.18±0.35 ng/ml), and the differences were statistically significant (t=7.184, 6.264, 3.836, 3.123, 9.128, all P < 0.05). The levels of Hb (119.56±22.25 g/ml), Alb (28.01±4.17 g/ml), eGFR (53.53±10.14 ml/min/1.73m2) and miR-346 (0.43±0.10 ) were lower than those in the good prognosis group(132.48±24.15 g/ml,34.22±5.82 g/ml,70.41±14.21 ml/min/1.73m2,0.71±0.15), and the differences were statistically significant (t=2.450, 5.426, 6.046, 9.615, all P<0.05). Logistic regression analysis showed that SCr,24-hour urinary protein quantitation,eGFR,miR-346, and SFRP-4 were independent influencing factors for poor prognosis in patients with IgA nephropathy ( all P<0.05). Conclusion Serum miR-346 level decreased and SFRP-4 level increased IgA nephropathy patients, which was closely related to the severity and prognosis of patients with IgA nephropathy.
