目的探讨内源性雌激素及雌激素受体α(ERα)水平与中老年男性冠心病的相关性。方法选择中日友好医院2012年心内科住院确诊的中老年男性冠心病患者98例,同时募集体检中心2012年9月~12月自愿加入实验研究的中老年男性健康体检者70例。所有研究对象均检测血清中6种内源性性激素水平,包括总睾酮(T)、雌二醇(E2)、黄体生成素(LH)、卵泡生成素(FSH)、孕酮(PROG)及泌乳素(PRL),并用流式细胞术检测外周血淋巴细胞雌激素受体α(ERα)水平。结果冠心病组E2水平低于对照组(84.71±34.31 pmol/L vs 67.44±30.56 pmol/L,t=2.626,P=0.02),其余激素水平差异均无统计学意义(均P>0.05)。两组ERα阳性率差异也无统计学意义(25.4%±9.3% vs 21.8%±10.4%,t=1.654,P>0.05)。Logistic回归分析显示E2降低冠心病发病风险(OR=0.91,95%CI 0.84~0.99,P=0.032)。E2与ERα阳性率之间呈正相关,而年龄与ER阳性率呈负相关。结论内源性雌激素水平减低与中老年男性人群冠心病发病密切相关。

ObjectiveTo investigate the correlation of endogenous estrogen and estrogen receptor-α (ERα) level with coronary artery diseases (CAD) in elderly males and explore the potential mechanism of gender factor in the prevalence of CAD.MethodsA total of 98 male patients with coronary artery diseases and 70 healthy male ages were enrolled.The serum levels of total testosterone,estradiol (E2),luteinizing hormone (LH),follicle-stimulating hormone (FSH),progestrone (PROG) and prolactin (PRL) were measured in these two groups.And the estrogen receptors alpha of peripheral lymphocytes were assessed by flow cytometry.ResultsThe serum level of E2 was lower in the CAD group than that in the control group (84.71±34.31 pmol/L vs 67.44±30.56 pmol/L,t=2.626,P=0.02).But two groups showed no statistic differences in the levels of T,LH,FSH,PROG,PRL and lymphocyte ERα (25.4%±9.3% vs 21.8%±10.4%,t=1.654,P>0.05).As demonstrated by the logistic regression analysis,the level of E2 was negatively correlated with the CAD risk (OR=0.91,95%CI 0.84~0.99,P=0.032) and positively correlated with the peripheral lymphocyte ER level.ConclusionThe deficiency of endogenous estrogen contributes to a high prevalence of CAD in elderly males.
