摘要:
目的 分析某院住院患者多重耐药菌(MDROs)的临床分布特征,为MDROs的预防与控制提供依据。方法 回顾性分析2012年1月~2014年12月该院MDROs的临床分布情况。结果 共监测住院病例116 316份,发生MDROs感染434例,感染率为0.37%,2014年(0.30%)与2012年(0.38%)比较,差异有统计学意义(χ2=3.91,P<0.05); 检出前五位多重耐药菌为大肠埃希菌(45.62%)、金黄色葡萄球菌(20.28%)、鲍曼不动杆菌(15.90%)、肺炎克雷伯菌(9.68%)和铜绿假单胞菌(8.53%); 前三位标本来源主要为痰液(58.29%)、尿液(19.59%)和其他分泌物标本(15.90%); 前三位科室为重症医学科(25.58%)、呼吸内科(23.04%)、泌尿外科(7.60%)。结论 不同科室MDROs临床分布不同,加强监测同时采取干预措施,能有效预防和控制MDROs感染的发生。
Abstract:
Objective To analyze the characteristics of clinical distribution of multidrug -resistant organisms(MDROs)in a hospital so as to put forward the intervention measures to prevent and control the MDROs infections.Methods Clinical distribution of MDROs was that isolated from patients in a hospital from Jan 1,2012 to Dec 31,2014 were analyzed retrospectively.Results 116 316 patients were admitted in this hospital of which 434(0.37%)cases of MDROs infections developed.The incidence of MDROs infectionswas 0.38% in 2012,0.30% in 2014,and there wasstatistically significant difference(χ2=3.91,P<0.05).The top five pathogens were Escherichia coli(45.62%),Staphylococcus aureus(20.28%),Bauman Acinetobacter(15.90%),Klebsiella Pneumoniae(9.68%),andPseudomonas aeruginosa(8.53%),respectively.The top three sample types were sputum(58.29%),urine(19.59%),and other secretions(15.90%),respectively.The top three departments were critical medical department(25.58%),respiratory department of internal medicine(23.04%)and department of urology(7.60%),respectively.Conclusion The clinical distribution characteristics of MDROs in different departments was different.It is an effective way to strengthen the monitoring of the MDROs infections and implement the intervention measures so as to prevent and control the MDROs infections.
