摘要:
目的 探讨联合检测血清纤维凝胶蛋白-3(Ficolin-3)、转铁蛋白(TRF)及超敏C反应蛋白(hs-CRP)对2型糖尿病(T2DM)的早期诊断价值。方法 选取深圳福田人民医院2013年10月~2015年10月收治的T2DM前期患者80例[空腹血糖受损(IFG)35例,糖耐量减低(IGT)45例]为A组,另外选取健康体检人群55例为B组,测定研究对象血清Ficolin-3,TRF及hs-CRP水平,评估其对于T2DM的早期诊断价值。结果 A组患者Ficolin-3,TRF及hs-CRP水平明显高于B组(27.4±2.5 μg/ml vs 18.9±2.3 μg/ml,277±12.7 mg/dl vs 219±9.8 mg/dl,3.68±0.74 mg/ml vs 1.23±0.83 mg/ml),差异具有统计学意义(P<0.05); 单独检测,对早期T2DM诊断的特异度和阳性预测值由高到低依次为TRF>Ficolin-3>hs-CRP(63.7%>54.5%>36.4%,72.2%>70.5%>50.7%),敏感度和阴性预测值由高到低为Ficolin-3>TRF>hs-CRP(70.0%>65.0%>45.0%,60.0%>55.6%>31.3%); 以串联方式检测,检测Ficolin-3,TRF及hs-CRP对早期T2DM的诊断特异性显著提高(89.5%),以并联方式检测,检测Ficolin-3,TRF及hs-CRP对早期T2DM的诊断敏感性显著提高(94.2%)。结论 联合检测血清Ficolin-3,TRF及hs-CRP对T2DM早期诊断具有重要价值,可有效提高早期T2DM诊断的特异度和灵敏度,值得临床推广。
Abstract:
Objective To explore the early diagnosticvalue of combined detection of Ficolin-3,TRF and hs-CRP for type 2 diabetes(T2DM).Methods 80 cases of pre-T2DM patients in Shenzhen Futian People's Hospital were collected as group A [impaired fasting glucose(IFG)35 cases of impaired glucose tolerance(IGT)45 cases)] from October 2013 to October 2015,55 cases of healthy people were incontrol group B.Serum Ficolin-3,TRF and hs-CRP levels of patients in two were test,evaluate the earlydiagnostic value of type 2 diabetes(T2DM).Results TheFicolin-3,TRF and hs-CRP levels of patients in A group were significantly higher than those in group B(27.4±2.5 μg/ml vs 18.9 ± 2.3 μg/ml,277±12.7 mg/dl vs 219±9.8 mg/dl,3.68±0.74 mg/ml vs 1.23±0.83 mg/ml),the difference was statistically significant(P<0.05).Individually tested,specificity and positive predictive value of early diagnosis of T2DM in descending order of TRF>Ficolin-3>hs-CRP(63.7%> 54.5%>36.4%,72.2%>70.5%>50.7%),the sensitivity and negative predictive value from high to low Ficolin-3>TRF>hs-CRP(70.0%>65.0%>45.0%,60.0%>55.6%>31.3%).In series testing,testFicolin-3,TRF and hs-CRP for early diagnosis of T2DM significantly increasedsignificantly(89.5%),in parallel testing,the sensitivity of testing Ficolin-3,TRF and hs-CRP for early diagnosis of T2DM improved significantly(94.2%).Conclusion Combined detection of serum Ficolin-3,TRFand hs-CRP is valuable for the early diagnosis of type 2 diabetes,and can improve the specificity and sensitivity of early diagnosis of T2DM effectively.It should be popularized.
