摘要:
目的 研究慢性肾脏疾病(CKD)贫血患者血清中性粒细胞明 胶酶相关脂质运载蛋白(NGAL)水平与CKD患者缺铁状态的临床关系。方法 〖HT5 ”K〗筛选154例CKD患者,分为贫血组和非贫血组,比较血清铁状态指标、红细胞参数、NGA L等的水平;CKD贫血患者中,按转铁蛋白饱和度(TSAT)分组,进行单变量相关性和多元回归 分析,以及ROC曲线分析,评估和比较CKD患者中血清NGAL、铁蛋白(Fer)、促红细胞生成素( EPO)和转铁蛋白受体(Tfr)的临床诊断潜力。结果 CKD贫血患者血清 NGAL水平明显高于无贫血的CKD患者(192.5±65.5 ng/ml vs 256.9±130.4 ng/ml,t =3.2,P<0.01);在单变量分析中,血清NGAL与血红蛋白(Hb)、血细胞比积(HCT)、红细 胞平均体积(MCV)、红细胞平均血红蛋白含量(MCH)、血清铁(Fe)、TSAT相关系数分别为-0. 36,-0.32,-0.26,-0.31,-0.38和-0.3。ROC曲线分析中,血清NGAL水平与CKD贫血 患者缺铁状态的曲线下面积(AUC)为0.713,灵敏度为58.6%(95%CI为 36.9~76.6), 特 异度为74.7%(95%CI为64.0~83.6)。结论 CKD缺铁性贫血患者 中,血清NGAL水平有可能成为评估缺铁状态和疗效的新指标。
Abstract:
Objective To evaluated the association between the NAGL and the chronic kidney disease (CKD) patients with iron status,which w as occurred iron scarcity for CKD patient in clinical.Methods 〖WT5 ”BZ〗154 CKD patients were included in this study.Serum ne utrophil gelatinase-related lipid transport protein(NGAL) were detected in anem ia group and non-anemia group of chronic kidney disease.The NGAL,univariate c orrelation and multivariate regression analysis,as well as ROC curve analysis, were used to assess and compared the diagnostic potential of NGAL,ferritin,EPO and Tfr (transferrin receptor) in CKD patients after analyzing the patient’s ir on status.Results NGAL levels were markedly higher in CK D patients with anemia than CKD patients without anemia.In the analysis,NGAL w a s inverse correlated with hemoglobin,hematocrit,MCV,MCH,serum iron and TSAT( cor relation relation was -0.36,-0.32,-0.26,-0.31,-0.38,-0.3,respectivel y).ROC analysis was employed to assess and compare the diagnostic potentials of the NGAL,ferritin,EPO,and Tfr (transferritin receptor) in identifying this c ondition among CKD patients.The NGAL showed that the area under the curve of ir on deficiency in CKD patients (AUC) was 0.713,with good sensitivity and specif icity (58.6% vs 74.7%).Conclusion NGAL might be propo sed as a new tool in the assessment of iron deficiency and in the management of iron therapy for CKD patients.
