摘要:
目的 探讨血清维生素B12、同型半胱氨酸(Hcy)在脊髓亚急性联合变性(subacute combined degeneration of the spinal cord,SCD)中的诊断价值。方法 选择2014年4月~2016年7月期间陕西省人民医院收治住院的29例SCD患者及同期住院的非脊髓病变患者74例,采用化学发光免疫法及酶联免疫化学发光法检测所有研究对象的血清维生素B12及Hcy浓度,采用卡方检验比较两组间的浓度,并比较两种检验方法的灵敏度、特异度及诊断符合率等指标。结果 SCD组及对照组的血清Hcy和维生素B12浓度四分位数分别为49.41 μmol/L vs 12.36 μmol/L,44.35 pg/ml vs 166.75 pg/ml,差异具有统计学意义(U=343.50,800.00,P<0.05); 血清维生素B12与Hcy呈线性负相关关系(r=-0.248,P=0.012); 血清维生素B12及Hcy检测的灵敏度分别为55.2% vs 89.7%,差异具有统计学意义(χ2 =8.631,P=0.003),且前者的诊断符合率高于后者(69.9% vs 64.1%),差异有统计学意义(χ2 =4.175,P=0.041)。结论 在SCD的诊断中,血清Hcy及维生素B12的检测在SCD中具有一定诊断价值,且血Hcy水平可作为诊断SCD的一个更为敏感的指标,有利于提高对SCD的早期诊断。
Abstract:
Objective To discuss the diagnostic value of detecting plasma vitamin B12 and Homocysteine(Hcy)in subacute combined degeneration of the spinal cord(SCD).Methods Parents of SCD(n=29)and subjects(n=74)without spinal cord diseases were enrolled from the clinic of Shaanxi Provincial People's Hospital from April 2014 to July 2016.The plasma vitamin B12 and Hcy concentrations were detected using Chemiluminescence Immunoassay(CLIA)and enzyme-linked immuno sorbent assay(ELISA)respectively,and compared using chi-square between the SCD and control groups.Sensitivity rate,specific rate,and diagnose accordance rate were evaluated.Results The plasma homocysteine and vitamin B12 concentrationsin SCD and control groups were 49.41 μmol/L vs 12.36 μmol/L,44.35 pg/ml vs166.75 pg/ml,with significant differences(U=343.50,800.00,P<0.05).Negative correlation existed between the concentrations of vitamin B12 and Hcy(r=-0.248,P=0.012).The sensitivity rates of vitamin B12 and Hcy detections were 55.2% vs 89.7%,which had significant differences(χ2=8.631,P=0.003),and the diagnose accordance rate(69.9% vs 64.1%)of the former was higher(χ2=4.175,P=0.041).Conclusion The plasmaHcy and vitamin B12 concentration detections had definite diagnostic value in SCD,and Hcy had higher sensitive rate,which can improve the diagnostic rate ofSCD in the early stage.
