摘要:
目的 探讨氨基端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)和肌钙蛋白I(cardiac troponin I,cTnI)对心律失常的诊断价值,以及对心律失常不同疾病类型之间的鉴别诊断价值。方法 收集2016年5月~2017年4月贵州省人民医院心内科确诊的114例心律失常住院患者血清作为疾病组,并选择同期体检的健康者108例作为对照组,以化学发光法检测NT-proBNP和cTnI血清水平。对所有资料进行正态性和方差齐性检验,采用t检验比较心律失常疾病组和对照组NT-proBNP和cTnI血清水平差异,采用单因素方差分析比较室性早搏、心房扑动、心房颤动和室性心动过速等不同疾病类型中NT-proBNP和cTnI血清水平差异。结果疾病组与对照组在性别与年龄上差异无统计学意义(t=0.24,1.47,P>0.05)。疾病组与对照组比较,血清NT-proBNP(855.96±101.46 pg/ml vs 369.77±194.88 pg/ml),cTnI(1.357±0.43 ng/ml vs 0.037±0.015 ng/ml)浓度均升高,且血清NT-proBNP,cTnI升高水平与健康对照组比较差异具有统计学意义(t=2.29,5.68,均P<0.05)。在室性早搏、心房扑动、心房颤动和室性心动过速等不同心律失常疾病类型中,心房扑动患者的NT-pro BNP血清水平(1 427.07 pg/ml)和室性心动过速患者cTnI(2.52ng/ml)血清水平最高,在四类疾病中NT-pro BNP和cTnI血清水平有着不同的分布差异(均P<0.05)。结论 NT-proBNP和cTnI血清水平对心律失常的诊断,以及心律失常不同疾病类型的鉴别,具有一定的参考价值。
Abstract:
Abstract:Objective To investigate the diagnostic value of N-terminal pro-brain natriuretic peptide(NT-proBNP)and cardiac troponinI(cTnI)in cardiac arrhythmia.And the value of differential diagnosis between different types of arrhythmia.Methods The 114 patients with arrhythmia inpatients diagnosed were collected as the disease group and 108 healthy subjects were collected as the control group in Department of Cardiology,Guizhou Province People's Hospital from May 2016 to April 2017.The serum levels of NT-proBNP and cTnI were detected by chemiluminescence method.All the datawere tested for normality and homogeneity of variance.T test was usedto compare the serum levels of NT-proBNP and cTnI between patients with arrhythmia and control group.The level of serological differences of NT-ProBNP and cTnI in patients with ventricular premature beats,atrial flutter,atrial fibrillation and ventricular tachycardia were compared by One-way ANOVA.Results There was no significant differences in sex and age betweenthe disease group and the control group(t=0.24,1.47,all P> 0.05).Compared with the control group,the serum levels of NT-proBNP(855.96 ± 101.46 pg/ml vs 369.77 ± 194.88 pg/ml)and cTnI ± 0.43 ng/ml vs 0.037 ± 0.015 ng/ml)were increased,and the serum levels of NT-proBNP and cTnI were significantly higher than those in healthy control group(t=2.29,5.68,all P<0.05).The serum level of NT-pro BNP in atrial flutter patients(1 427.07pg / ml)and the serum level of cTnI(2.52 ng/ml)in ventricular tachycardia patients were the highest in premature ventricular contractions,atrial flutter, atrial fibrillation,ventricular tachycardia and other types of arrhythmia.There had different distribution(P<0.05)of NT-pro BNP and cTnI serum levelsamong the four kinds of diseases.Conclusion The serum levels of NT-pro BNP and cTnI have some reference value in the diagnosis of arrhythmia and in the differential diagnosis of different types of arrhythmia.
