摘要:
目的 探讨样本采集量、采集顺序、放置时间、温度及不同类型采血管对血钾测定的影响。方法 收集2017年10~12月自愿参加实验的健康体检者及患者76例,根据实验内容采集静脉血分组进行样本采集量、采集顺序、放置时间、温度及不同类型采血管血钾浓度差异比较,并计算回归方程。将血钾医学决定水平2.8,3.6,5.5和6.2 mmol/L代入回归方程,如果所有决定水平处偏差在3%以内,则认为血钾差异在临床允许范围内。结果 不同真空采血管血钾测定有差异,血浆钾结果低于血清钾结果,不同促凝剂对血钾结果可能有影响; 采血量及采血顺序对血钾测定影响小; 标本采集后8 h内检测对血钾结果影响不大; 存放温度对血钾影响大,37℃结果变低,4℃结果变高。结论 各实验室应对血钾测定的真空采血管选择、标本运送及保存温度等分析前因素进行规范,以确保血钾检测的分析前质量保证。
Abstract:
Objective To explore influential factors of blood potassium determination such as sample collection volume,collection order,standing time,temperature and different kinds of vacuum blood collection tube.Methods 76 patients who volunteered to take part in the experiment were enrolled from October to December 2017.The groups were created according to the experimental contents,such as the sample collection volume,thecollection order,the standing time,the temperature and the different kinds ofvacuum blood collection tube.The blood potassium concentration were compared and the regression formula was calculated.The medicine decide levels of blood potassium 2.8,3.6,5.5 and 6.2 mmol/L were replaced into the regression equation.If the bias of all the determination levels was within 3%,the difference of blood potassium was considered to be within the range of clinical permission.Results There was a difference for potassium level in different vacuum blood collection tube,the results of plasma potassium were lowerthan those of serum.The different coagulants might affect the results of bloodpotassium.Blood collection volume and blood collection order had little effectonblood potassium determination.Within 8 hours after specimen was collected,there was little effect on blood potassium determination.The temperature had a great effect on blood potassium determination,the results was lower at 37℃ and increased at 4℃.Conclusion In order to ensure the pre-analysis quality assurance of the blood potassium detection,the laboratory shouldstandardize the pre analysis factors such as the selection of the vacuum bloodcollection tube,the temperature of the delivery and preservation for the specimen.
