摘要:
目的 利用六西格玛(sigma,σ)性能验证图及质量目标指数(quality goal index,QGI)评价临床常规生化检测项目分析性能,指导实验室质量持续改进。方法 不精密度(coefficient of variation,CV%)采用2017年生化室常规化学检测项目室内质控累积变异系数,偏倚(bias%)采用2017年第一次参加卫生部室间质评计划中常规生化项目的百分差值,允许总误差(TEa%)采用国家标准GB/T20470-2006及卫生行业标准WS/T 403-2012质量规范,利用卫生部室间质评信息系统中西格玛性能验证功能绘制西格玛性能评价图,并计算QGI。结果 当使用国家标准时,σ< 3的只有氯1个项目,3≤σ<4有天门冬氨酸氨基转移酶和碱性磷酸酶2个项目,4≤σ<5有尿素和肌酐2个项目,5≤σ<6有乳酸脱氢酶1个项目,σ≥6有钾、钠、钙、镁、血糖、尿酸、总蛋白、清蛋白、总胆固醇、三酰甘油、丙氨酸氨基转移酶、肌酸激酶等12个项目; 当使用卫生行业标准时,σ< 3有氯、碱性磷酸酶和天门冬氨酸氨基转移酶3个项目,3≤σ<4有钠、清蛋白、尿素、肌酐、乳酸脱氢酶5个项目,4≤σ<5有钙、血糖、肌酸激酶3个项目,5≤σ<6有钾、镁、丙氨酸氨基转移酶3个项目,σ≥6有总蛋白、尿酸、总胆固醇、三酰甘油4个项目。结论 国家标准比行业标准宽松,西格玛性能验证采用行业标准更能有效的评价检测项目的分析性能以及导致分析性能不佳的原因,提出检测项目个性化的质控措施,提高实验室质控水平。
Abstract:
Objective Using the chart of the six sigma performance verification and the quality goal index(QGI)to evaluate the analysisperformance of clinical routine biochemical test projects,and improve laboratory quality continuesly.Methods Indensities(coefficientof variation,the CV%)were used in the indoor quality control accumulationvariation coefficient of the routine chemical detection project of biochemicalchamber in 2017,bias(by%)was used for the first time in 2017 to participatein the percentile value of routine biochemical projects in the health department,allow total error%(TEa)to adopt national standard GB/T20470-2006 and health industry standard WS/T 403-2012 quality standard,the performance evaluationgraph of sigma performance was drawn by using the sigma performance verificationfunction of the health department,Persia QGI.Results When the national standard was used,only one project of Cl was σ< 3,two projects of ALT and AKP was 3≤σ<4,two project of CRE and BUN was 4≤σ<5,and owe project of LDH was 5≤σ<6.The twelve projects'sigma greater than six were K,Na,Ca,Mg,GLU,UA,TP,ALB,TC,TG,ALT and CK.When health industry standards,Cl,ALP and AST was σ<3,and Na,ALP,BUN,LDH and CRE was 3≤σ<4,and Ca,GLU and CK was 4≤σ<5,and K,Mg,ALT was 5≤σ<6.The four projects'sigma greater than six were three TP,TC,TG and UA.Conclusion National standards are looser than industry standards,and Sigma performance verification can use industry standard evaluation test performance more effectively and analyze the cause of the poor performance.The test items personalized qualitycontrol measures are put forward,which could improve the quality control of laboratory.
