目的探讨血清超敏C-反应蛋白(hypersensitive C-reac tive protein,hs-CRP),β-人绒毛膜促性腺激素(β-human chorionic gonadotropin ,β-HCG)和孕酮(Progesterone,P)的联合检测在预测早期先兆流产中的临床应用。 方法选择2017年1~12月孕龄为8~12周并发生先兆流产的61例孕妇为研 究对象,选择同期孕龄相同的72例正常妊娠孕妇为对照组。采用化学发光法检测血清β-HC G和P的水平,并使用增强型免疫比浊法检测血清hs-CRP水平。通过Logistic回归分析先兆 流产的独立危险因素,并以ROC曲线检测三项指标对预测先兆流产的效能。结果 正常妊娠组血清P,β-HCG水平分别为27.76±3.38 ng/ml和8 886±1 303 mIU/ml,均明显高于先兆流产组(19.62±2.58 ng/ml,6 338±1 461 mIU/ml),差异具有 统计学意义( t=15.42,10.63,均P <0.000 1);而正常妊娠组血清hs-CRP水平(5.9 5±2.19 mg/L)明显低于先兆流产组(14.91±6.55 mg/L),差异具有统计学意义( t=10 . 92,P <0.000 1)。单变量或多变量Logistic回归分析发现,血清hs-CRP,P和β-HCG均 是先兆流产的独立影响因素( OR=2.060,0.408,0.999,均P <0.000 1)或( OR=1 .803,0.389,0.998,均P <0.05)。血清hs-CRP,β-HCG和P诊断先兆流产的受试者工作曲线(receiver operating characteristic curve,ROC)曲线下面积分别为0.954,0 .972和0.911(均 P <0.000 1);三项目联合检测的ROC曲线下面积为0.997( P <0.000 1)。结论联合检测血清hs-CRP,β-HCG和P的水平,在早期诊断先兆流产的发生中具有重要临床应用价值。
Abstract:
Objective To study the combined detection value of serum hypersensitive C-reactive protein (hs-CRP),β-human chorionic gona dotropin (β-HCG),and progesterone (P) in the prediction of threatened abortio n.Methods A total of 61 pregnant women with threatened a bortion from January to December 2017 were selected as research subjects in this study,and 72 normal pregnant women were selected as control group.All pregnan t women had a gestational age of 8~12 weeks.The serum P and β-HCG of all gro ups were detected by chemiluminescence immunoassay,and serum hs-CRP was analyz ed by enhanced immuno-turbidimetric method.Logistic regression analyzed the in dependent risk factors for threatened abortion,and ROC curve was used to predic t the efficacy of three markers in threatened abortion.Results The serum β-HCG and progesterone of normal control were 27.76±3.38 n g/ml and 8 886±1 303 mIU/ml,which was higher than those of threatened abortion (19 .62±2.58 ng/ml,6 338±1 461 mIU/ml),and there was significant difference ( t =15.42,10.63,all P <0.000 1).The serum hs-CRP level in normal grou p (5.9 5±2.19 mg/L) was significantly lower than the group with threatened abortion (14.91±6.55 mg/L) and there was significant difference ( t=10.92,P <0. 000 1).Univariate or multivariate logistic regression analysis found that seru m hs-CRP,P and β-HCG were independent influencing factors of threatened abor tion ( OR =2.060,0.408 and 0.999,all P <0.000 1) or ( OR =1.803, 0.389 and 0.998,all P <0.05).The area under the receiver operating chara cteristic curve (ROC) of serum hs-CRP,β-HCG and P were 0.954,0.972 and 0.911,respective ly (all P <0.000 1).And the AUC of the combination of three markers was 0 .997 ( P <0.000 1).Conclusion The combined detectio n of serum hs-CRP,β-HCG and P in the prediction of early threatened abortion has a great clinical value.
