摘要:
目的 探讨外周血Th17,Treg细胞亚群和细胞因子在儿童支原体肺炎发生发展中的变化以及检测外周血C-反应蛋白(CRP)和降钙素原(PCT)的临床意义。方法 收集2017年1月~2018年8月来解放军第901医院儿科确诊的支原体(MP)感染性肺炎患儿75例作为实验组,纳入标准为病程均在一周以内并且排除其他疾病,ELISA方法检测血清特异性MP-IgM≥1:40,选择同期健康体检儿童75例作为对照组。采用流式细胞术检测两组外周血Th17和Treg细胞亚群比例,采用ELISA方法检测两组外周血细胞因子IL-17和IL-10的表达水平,采用散射比浊法检测两组血清CRP水平,采用化学发光法检测两组血清PCT水平,实验组治疗一周后再次抽血检查CRP和PCT水平。结果 Th17细胞,Treg细胞,IL-17,IL-10,CRP和PCT水平均采用均数±标准差((-overx)&#177;s)表示,实验组检测结果依次为3.35%±1.26%,3.39%±1.04%,19.12±2.96 pg/ml,8.94±0.87 pg/ml,21.58±2.78 mg/L和0.32±0.05 ng/ml,对照组检测结果依次为1.60%±0.29%,7.48%±1.28%,9.59±2.06 pg/ml,22.16±1.09 pg/ml,2.41±0.39 mg/L和0.05±0.01 ng/ml,实验组治疗一周后CRP和PCT检测结果依次为7.38±0.46 mg/L,0.15±0.02 ng/ml。实验组外周血Th17细胞的百分比、IL-17的表达水平、CRP和PCT水平均高于对照组,差异均具有统计学意义(t值分别为3.03,3.36,6.54,5.16,均P<0.05); 实验组外周血Treg细胞的百分比和IL-10的表达水平均低于对照组,差异具有统计学意义(t值分别为3.68,4.07,均P<0.05); 与治疗前相比,实验组治疗一周后外周血CRP和PCT水平均降低,差异具有统计学意义(t值分别为4.06,3.15,均P<0.05)。结论 支原体肺炎患儿外周血Th17细胞增加,Treg细胞减少,TH17/Treg比例失衡,由其分泌的细胞因子表达异常,可能在儿童支原体肺炎的发生发展中起着重要作用。联合检测CRP和PCT在支原体肺炎患儿病情观察和疗效判断上具有重要价值。
Abstract:
Objective To investigate the changes of peripheral blood TH17,Treg cell subsets and cytokines in the development of childhoodMycoplasma pneumonia(MP)and the clinical significance of detecting peripheral blood C-reactive protein(CRP)and procalcitonin(PCT).MethodsSeventy-five children with mycoplasma(MP)-infected pneumonia diagnosed in the pediatric department of the People's Liberation Army No.901 Hospital from January 2017 to August 2018 were enrolled as the experimental group.The inclusion criteria were within one week and other diseases were excluded.ELISA was used to detect the disease.Serum-specific MP-IgM≥1:40,75 children in the same period of healthy physical examination were selected as the control group.The ratio of Th17 and Treg cell subsets in peripheral blood was detected by flow cytometry.The expression levels of cytokines IL-17 and IL-10 in peripheral blood were detected by ELISA.Serum CRP levels were detected by turbidimetry.The PCT levels of serum were detected by chemiluminescence method.The levelsof CRP and PCT in the experimental group were examined again after one week oftreatment.Results Th17 cells,Treg cells,IL-17,IL-10,CRP and PCT were expressed as mean±standard deviation((-overx)&#177;s).Theexperimental results were 3.35%±1.26%,3.39%±1.04%,19.12±2.96 pg/ml,8.94±0.87 pg/ml,21.58±2.78 mg/L and 0.32±0.05 ng/ml,respectively,andthe control results of the control group were 1.60%±0.29%,7.48%±1.28%,9.59±2.06 pg/ml,22.16±1.09 pg/ml,2.41±0.39 mg/L and 0.05±0.01 ng/ml,respectively.One week after treatment in the experimental group the CRP and PCT test results were 7.38±0.46 mg/L,0.15±0.02 ng/ml,respectively.The percentage of peripheral blood Th17 cells,IL-17 expression,CRP and PCT levels in the experimental group were higher than those in the control group,and the difference was statistically significant(t values were 3.03,3.36,6.54 and 5.16,all P<0.05).The percentage of peripheral blood Treg cells and IL-10 expression in the experimental group were lower than those in the control group,and the difference was statistically significant(t values were 3.68and 4.07,all P<0.05).Compared with before treatment,the experiment peripheral blood CRP and PCT levels decreased after one week of treatment,and thedifference was statistically significant(t values were 4.06 and 3.15,allP<0.05).Conclusion The peripheral blood Th17 cellsof children with mycoplasma pneumonia increase,the Treg cells decrease,the TH17/Treg ratio was imbalanced,and the abnormal expression of cytokines secretedby them may play an important role in the development of childhood mycoplasma pneumonia.Combined detection of CRP and PCT Children with mycoplasma pneumonia have important value in disease observation and efficacy judgment.
