摘要:
目的 探讨食管癌患者血栓弹力图(thromboelastography,TEG)与常规凝血功能参数之间的相关性。方法 选择2018年7月~2019年5月在南充市中心医院确诊的108例食管癌患者作为研究对象,入院后同时检测TEG、常规凝血功能及血细胞计数,并对检测结果进行相关回归分析。采用χ2检验比较TEG与常规凝血功能参数异常率的差异。结果 相关性分析显示食管癌患者的TEG参数中凝血反应时间(R)与活化部分凝血酶时间(APTT)和血细胞凝集块形成时间(K)呈正相关,与D-二聚体(D-D)、纤维蛋白(原)降解产物(FDPs)、血细胞凝块形成速率(α-Angle)、血凝块最大强度(MA)和凝血综合指数(CI)呈负相关; K与纤维蛋白原(FIB),D-D,FDPs,血小板计数(PLT),α-Angle,MA,CI呈负相关; α-Angle与FIB,D-D,FDPs,PLT,MA和CI呈正相关,与凝血酶时间(TT)呈负相关; MA与D-D,FIB,PLT和CI呈正相关; CI与FIB,D-D,FDPs和PLT呈正相关,与APTT和TT呈负相关。TEG检测与常规凝血功能检测异常率差异无统计学意义(P>0.05)。结论 TEG与常规凝血功能参数之间存在一定的相关性,结果具有一致性,二者不能相互替代,可及时为食管癌患者的凝血状态评估提供依据。
Abstract:
Objective To investigate the correlation between thromboelastography(TEG)and conventional coagulation parameters in patientswith esophageal cancer.Methods A total of 108 patients with esophageal cancer diagnosed in the Central Hospital of Nanchong from July 2018 to May 2019 were enrolled.TEG,routine coagulation function and blood test were detected at the time of admission.Linear correlation and regression analysis was performed among these parameters.The χ2 test was used to compare the difference between the abnormal rate of TEG and conventional coagulation parameters.Results Correlation analysis showed that the R value in the TEG parameters of patients with esophageal cancer was positively correlated with APTT and K,and negatively correlated with D-D,FDPs,α-Angle,MAand CI.K was negatively correlated with FIB,D-D,FDPs,PLT,α-Angle,MA and CI.α-Angle was positively correlated with FIB,DD,FDPs,PLT,MA and CI,and negatively correlated with TT.MA was positively correlated with DD,FIB,PLTand CI.CI was positively correlated with FIB,DD,FDPs and PLT,and negativelycorrelated with APTT and TT.There was no significant difference in the abnomalrate between TEG and conventional coagulation(P>0.05).Conclusion There were significant correlation between TEG and conventional coagulation parameters,and the results were consistent.The two methods can notreplace each other,which can provide the basis for the evaluation of coagulation status in patients with esophageal cancer.
