摘要:
目的 分析哮喘- 慢阻肺重叠综合征(ACOS)患者辅助性T 细胞17(Th17)和调节性T 细胞(Treg)表达水平,及其与肺功能的相关性。方法 采用流式细胞术检测60 例ACOS 患者(ACOS 组)、60 例哮喘患者(哮喘组)和60 例慢性阻塞性肺疾病患者(COPD 组)外周血Th17 和Treg 细胞的表达,比较分析其与肺功能指标的关系。结果 与哮喘组比较,COPD 组、ACOS 组外周血CD4+ 细胞中Th17 细胞表达及Th17/Treg 比例明显升高,而Treg细胞表达明显下降,差异均有统计学意义 (F=15.36~36.61,均P<0.05);与COPD 组比较,ACOS 组外周血CD4+ 细胞中Th17 细胞表达及Th17/Treg 比例明显升高,而Treg 细胞表达明显下降,差异有统计学意义 (F=36.61,21.18,15.26, P<0.05)。ACOS 组患者FVC%pred,FEV1%pred 和FEV1/FVC 明显低于哮喘组,差异均有统计学意义(F=7.36,11.36,9.68, P<0.05),但FVC%pred 与COPD 组患者比较,差异无统计意义(t=0.52, P>0.05)。ACOS 患者外周血Th17 和h17/Treg 表达与FVC%pred,FEV1%pred 和FEV1/FVC 呈显著负相关(r=-0.502~-0.407, 均P<0.05)。Treg与FVC%pred,FEV1%pred 和EV1/FVC 呈显著正相关(r=0.019~0.033, 均P<0.05)。结论 ACOS 患者外周血Th17 和Treg 细胞表达异常,存在Th17/Treg 比例失衡,且与肺功能下降密切相关。
