摘要:
目的 探讨精索静脉曲张（VC）患者精浆转化生长因子-β（TGF-β）, 白介素-2（IL-2）和白介素-6（IL-6）水平与精液质量的相关性。方法 选取江苏省兴化市第五人民医院2018 年1 月~2019 年12 月收治的40 例VC 患者（观察组），同时抽取同期体检健康者40 例为对照组，采用酶联免疫吸附试验（ELISA）检测两组研究对象精浆TGF-β, IL-2 和IL-6 等指标水平，同时采集精液进行精液质量常规检查，评价TGF-β, IL-2 和IL-6 等指标水平与VC患者精液质量的相关性。结果 观察组患者采集的精液量和精液pH 值分别为3.14±0.76ml 和7.30±0.15，对照组分别为2.97±0.81ml 和7.29±0.16, 两组比较差异无统计学意义（t=0.968，0.288，均P ＞ 0.05），观察组患者精子密度、精子活动率、IL-2, IL-6 和TGF-β 分别为（34.65±9.02）×106/ml，(51.35±7.21)%，0.43±0.11ng/ml，0.53±0.08ng/ml 和138.61±24.92ng/ml； 对照组分别为(51.23±6.23)×106/ml, (68.04±5.16)%, 1.18±0.15ng/ml, 1.42±0.16ng/ml 和201.64±53.21ng/ml，组间比较差异具有统计学意义（t=9.566，11.906，25.501，31.466 和6.485, 均P ＜ 0.05）。精浆中的IL-2, IL-6，TGF-β 水平和VC 患者的精子密度、精子活动率呈明显的负相关性（r=-0.865 ～ -0.821, 均P ＜ 0.05），IL-2, IL-6 和TGF-β 之间呈明显的正相关性（r=0.045 ～ 0.051, 均P ＜ 0.05），与患者精液量和精液pH 值无明显相关性。结论 VC 患者IL-2, IL-6 和TGF-β 等细胞因子水平高低与精液质量呈明显的负相关，为阐明VC 不育机制和临床治疗提供依据。
