摘要:
目的 分析创伤性湿肺并发急性呼吸窘迫综合征(ARDS)纤溶患者血清中NF-KB p65 表达特点。方法 2017 年11 月~2018 年11 月选取宝鸡市中心医院胸外科收治的创伤性湿肺并发ARDS 患者62 例,设为观察组;另选取同期收治的62 例创伤性湿肺患者,设为参考组;选取同期收治的62 例轻微肺部损伤患者,设为对照组。入院和治疗1 周后,对比三组血NF-KB p65 表达、纤维蛋白溶解情况、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)和纤维蛋白原(FIB)水平变化情况。结果 入院时,观察组、参考组和对照组血NF-KB p65表达阳性率分别为98.39%,67.74% 和9.68%,观察组血NF-KB p65 表达阳性率高于参考组和对照组(χ2=6.023,11.276),参考组高于对照组(χ2=9.120),差异均有统计学意义(均P<0.05)。观察组纤维蛋白原含量低于参考组和对照组,参考组纤维蛋白原含量低于对照组,观察组凝血酶原时间、活化部分凝血活酶时间和凝血酶时间高于参考组和对照组,参考组凝血酶原时间、活化部分凝血活酶时间和凝血酶时间高于对照组。治疗1 周后,观察组、参考组和对照组血NF-KB p65 表达阳性率分别为11.29%,9.68% 和9.68%,三组血NF-KB p65 表达阳性率差异无统计学意义(χ2=0.062~0.206,均P>0.05);三组纤维蛋白原含量、凝血酶原时间、活化部分凝血活酶时间和凝血酶时间对比,差异均无统计学意义(均P>0.05)。结论 创伤性湿肺并发ARDS 患者,具有血NF-KB p65 表达升高、纤维蛋白原含量下降和凝血功能异常的特征。血NF-KB p65 表达可作为评析患者肺部疾病进展和危重程度的可靠指标。
