摘要:
目的　探究多囊卵巢综合征(polycystic ovary syndrome, PCOS)患者血清镍纹样蛋白(meteorin-like protein, Metrnl)水平检测的临床意义及与实验室指标的相关性。方法　 选取2018年4月～2019年9月西安高新医院妇科收治的61例PCOS患者为观察组，另选取同期58例健康体检女性作为对照组。应用全自动生化分析仪检测两组的总胆固醇(total cholesterol, TC)、三酰甘油(triglyceride, TG)、空腹血糖(fasting blood glucose, FBG)、超敏C反应蛋白(hypersensitive C-reactive protein, hs-CRP)、游离睾酮(free testosterone, FT)、卵泡刺激素(follicle-stimulating hormone, FSH)和促黄体生成素(luteinizing hormone, LH)等水平。采用酶联免疫吸附法(ELISA)检测血清中Metrnl的水平。相关性分析采用Spearson相关性检验。比较两组的实验室指标，正常和肥胖人群中血清Metrnl水平，分析Metrnl表达水平与实验室指标的相关性。结果　两组的FBG，TC，低密度脂蛋白胆固醇(low density lipoprotein-cholesterin, LDL-C)和高密度脂蛋白胆固醇(high density lipoprotein-cholesterol, HDL-C)等实验室指标差异均无统计学意义(t=0.12，1.95，0.87，0.48，均P>0.05)，观察组的hs-CRP和FT水平高于对照组，观察组的Metrnl (98.4±32.2 pg/ml)显著高于对照组(65.3±27.7 pg/ml)，差异具有统计学意义(t=5.36，13.20，5.91，均P<0.05)。对照组中正常(18.5 kg/m2≤BMI<24 kg/m2)和肥胖(BMI≥24 kg/m2)人群血清Metrnl水平差异无统计学意义(t=3.70，P>0.05)。观察组中肥胖人群血清Metrnl水平显著高于正常人群，差异具有统计学意义(t=11.41，P＜0.05)。Metrnl表达水平与hs-CRP，FT和BMI呈正相关关系(r=0.507，0.309和0.391，均P =0.000)。结论　Metrnl可作为PCOS潜在的生物学标记物，其表达水平与患者的hs-CRP，FT和BMI呈正相关。
