摘要:
目的 探讨血清泛连接蛋白1(Pannexin1)、可溶性髓系细胞触发受体1(soluble triggering receptors expressed onmyeloid cells-1,sTREM-1)在老年急性脑梗死(acute cerebral infarction)中的表达及其与病情、预后的关系。方法 选取2019 年6 月~ 2020 年8 月期间在辽阳市中心医院接受治疗的老年急性脑梗死患者126 例作为脑梗死组,根据患者的病情严重程度将其分为轻度组、中度组和重度组,另根据患者的预后情况将其分为预后良好组和预后不良组;另选取同期健康志愿者50 例作为健康对照组。收集所有研究对象的一般资料,采用酶联免疫吸附法检测血清Pannexin1 和sTREM-1 的水平。结果 脑梗死组的血清Pannexin1(4.52±2.36mg/ml )和sTREM1 水平(25.89±7.62pg/ml)高于健康对照组(2.61±0.98mg/ml,11.52±5.38pg/ml),差异均有统计学意义(t=5.529,12.175,均P =0.000)。重度组、中度组和轻度组的血清Pannexin1(5.68±2.48mg/ml, 4.62±2.15mg/ml,3.28±1.26mg/ml)和sTREM-1 水平(31.00±8.82pg/ml ,26.02±8.01pg/ml,20.87±6.58pg/ml )逐渐降低,差异有统计学意义(F=12.956,15.275,均P =0.000)。预后不良组的血清Pannexin1(6.00±2.63mg/ml)和sTREM-1 水平(33.73±9.42pg/ml)高于预后良好组(3.78±1.31mg/ml,21.97±7.15pg/ml),差异有统计学意义(t=6.338 ,7.805,均P =0.000)。血清Pannexin1 和sTREM-1 预测急性脑梗死患者预后不良的ROC 曲线下面积分别为0.767 和0.811。二者联合后敏感度为90.48%,较单一指标的敏感度有了大幅度提高,且约登指数也提高至0.619。结论 血清Pannexin1 和sTREM-1 的表达水平与老年急性脑梗死患者的病情严重程度以及预后密切相关,二者联合对患者的预后有较高的预测价值。
Abstract:
Objective To investigate the expression of serum Pannexin1 and soluble triggering receptors expressed on myeloid cells-1 (sTREM-1) in elderly patients with acute cerebral infarction and its relationship with the condition and prognosis. Methods 126 elderly patients with acute cerebral infarction who were treated in the Central Hospital of Liaoyang City from June 2019 to August 2020 were selected as the cerebral infarction group, and the patients were divided into mild group, moderate group and severe group according to the severity of their illness. According to the prognosis of patients, they were divided into good prognosis group and poor prognosis group;In addition, 50 healthy volunteers who had a physical examination in our hospital during the same period were selected as the healthy control group.The general data of all subjects were collected, and the levels of serum Pannexin1 and sTREM-1 were detected by enzyme-linked immunosorbent assay. Results The levels of serum Pannexin1 (4.52±2.36mg/ml ) and sTREM-1 (25.89±7.62pg/ml ) in the cerebral infarction group were higher than those in healthy control group(2.61±0.98mg/ml,11.52±5.38pg/ml ), the differences were statistically significant (t=5.529,12.175, all P=0.000).The levels of serum Pannexin1 (5.68±2.48mg/ml, 4.62±2.15mg/ml, 3.28±1.26mg/ml) and sTREM-1 (31.00±8.82pg/ml, 26.02±8.01pg/ml, 20.87±6.58pg/ml ) in severe group, moderate group and mild group were decreased gradually,the differences were statistically significant (F=12.956, 15.275, all P=0.000).The levels of serum Pannexin1 (6.00±2.63mg/ml) and sTREM-1 (33.73±9.42pg/ml) in the poor prognosis group were higher than those in the good prognosis group(3.78±1.31mg/ml, 21.97±7.15pg/ml), the differences were statistically significant (t=6.338, 7.805, all P=0.000).The area under the ROC curve of serum Pannexin1 and sTREM-1 predicting poor prognosis of patients with acute cerebral infarction was 0.767 and 0.811, respectively. The sensitivity of the combination of the two was 90.48%, which was significantly higher than the sensitivity of a single indicator, and the Youden index had also increased to 0.619. Conclusion The expression levels of serum Pannexin1 and sTREM-1 were closely related to the severity and prognosis of elderly patients with acute cerebral infarction, and the combination of the two has a high predictive value for the prognosis of patients.
