摘要:
目的 探究IgA 血管炎(IgAV) 儿童外周血ATP 合成酶β5 亚基(ATP5B)mRNA,泛素蛋白A52 残基核糖体蛋白融合产物(UBA52)mRNA 表达与肾损伤及疾病复发的关系。方法 选取2022 年1 月~ 2023 年10 月上海市奉贤区中心医院收治的143 例IgAV 患儿作为研究对象,根据是否并发累及肾脏进一步分为IgAV 肾损伤组(n=82)和IgAV 肾非损伤组(n=61);根据术后随访情况将病例分为复发组(n=44)和未复发组(n=99);另选取同期60 例健康体检者为对照组。实时荧光定量PCR(qRT-PCR)法检测ATP5B mRNA,UBA52 mRNA 表达水平,对比两者在各组中表达情况。多因素Logistic 回归分析IgAV 患儿发生肾损伤的影响因素;通过受试者工作特征(ROC) 曲线分析ATP5BmRNA,UBA52 mRNA 表达对IgAV 患儿发生肾损伤及疾病复发的预测价值。结果 对照组、IgAV 肾非损伤组和IgAV肾损伤组外周血中ATP5B mRNA(1.01±0.21,1.24±0.26,1.46±0.35),UBA52mRNA(1.03±0.19,1.19±0.22,1.42±0.28),Scr(55.69±12.78μmol/L,62.85±13.92μmol/L,76.49±15.34μmol/L),BUN(11.85±2.91mmol/L,13.46±2.78mmol/L,17.54±3.45mmol/L)水平依次升高,组间比较差异具有统计学意义(F=39.666 ~ 64.417,均P<0.05)。与未复发组比较,复发组外周血中ATP5B mRNA(1.52±0.34 vs 1.19±0.20),UBA52 mRNA(1.49±0.31 vs1.08±0.21) 表达升高,差异具有统计学意义(t=7.253,9.241,均P<0.001)。Logistic 回归分析显示,Scr,BUN,ATP5BmRNA,UBA52 mRNA 是IgAV 发生肾损伤的独立危险因素( 均P<0.05)。外周血中ATP5B mRNA 联合UBA52 mRNA预测IgAV 患儿发生肾损伤的AUC(95%CI) 为0.823(0.607 ~ 0.914),明显高于两者单独预测(Z=1.952,2.021,均P<0.05);外周血中ATP5B mRNA 联合UBA52 mRNA 预测IgAV 疾病复发的AUC(95%CI) 为0.851(0.617 ~ 0.958),亦明显优于两者单独预测(Z=2.035,2.098,均P<0.05)。结论 IgAV 患儿外周血中ATP5B mRNA,UBA52 mRNA表达水平与IgAV 肾损伤和复发相关,两者可作为疾病诊断和预后评估的有效生物标志物。
Abstract:
Objective To explore the relationship between the expression of ATP5B mRNA and UBA52 mRNA in the peripheral blood of children with IgA vasculitis(IgA) and kidney injury and disease recurrence. Methods 143 children with IgAV admitted to Shanghai Fengxian District Central Hospital from January 2022 to October 2023 were selected as the study subjects, and were further divided into IgAV kidney injury group(n=82)and IgAV kidney non-injury group (n=61)according to whether they were combined with the involvement of the kidneys . According to the postoperative follow-up, the patients were divided into a relapse group (n=44)and a non-recurrence group (n=99). In addition, 60 cases of healthy medical checkups were selected as the control group in the same period. Real-time fluorescence quantitative PCR was used to detect the expression levels of ATP5B mRNA and UBA52mRNA and compared the expression levels of the two in each group. Multivariate Logistic regression analysis was performed to analyze the influencing factors of kidney injury in IgAV children. The predictive value of ATP5BmRNA and UBA52mRNA expression on renal injury and disease recurrence in IgAV children was analyzed by receiver operator characteristic (ROC) curve. Results The levels of ATP5BmRNA(1.01±0.21,1.24±0.26,1.46±0.35), UBA52 mRNA (1.03±0.19, 1.19±0.22, 1.42±0.28), Scr(55.69±12.78μmol/L, 62.85±13.92μmol/L, 76.49±15.34μmol/L) and BUN(11.85±2.91mmol/L, 13.46±2.78mmol/L, 17.54±3.45mmol/L) in peripheral blood of the control group, IgAV renal injury group and IgAV renal injury group were increased successively, and the differences between groups were statistically significant(F=39.666 ~ 64.417,all P<0.05). Compared with the non-relapsed group,the expressions of ATP5B mRNA (1.52±0.34 vs 1.19±0.20)and UBA52 mRNA (1.49±0.31 vs 1.08±0.21) in peripheral blood of the relapsed group were increased, and the differences were statistically significant (t=7.253,9.241,all P<0.001). Logistic regression analysis showed that,Scr,BUN,ATP5B and UBA52 were independent risk factors for the development of renal injury in IgAV (all P<0.05). The AUC(95%CI) of ATP5BmRNA combined with UBA52mRNA in peripheral blood was 0.823(0.607 ~ 0.914), which was significantly higher than the single prediction of both(Z=1.952,2.021,all P<0.05). The AUC(95%CI) of ATP5BmRNA combined with UBA52mRNA in peripheral blood to predict IgAV disease recurrence was 0.851(0.617 ~ 0.958), which was also significantly better than the single prediction of both(Z=2.035,2.098,all P<0.05). Conclusion The expression levels of ATP5B mRNA and UBA52 mRNA in peripheral blood of children with IgAV are associated with renal injury and recurrence of IgAV, which can be used as effective biomarkers for disease diagnosis and prognosis.
