摘要:
目的?探讨血清自身抗体谱在抑郁症患者中的表达特征及与抑郁症患者预后的相关性。方法选取2018年7月~2024年3月南京医科大学附属脑科医院诊治的3503例抑郁症患者作为研究对象,采用免疫印迹法(Westernblotting)检测血清自身抗体谱的表达,并收集患者临床资料。选取资料完整的115例血清自身抗体谱阳性的抑郁症患者作为观察组,以同期86例血清自身抗体谱阴性的抑郁症患者作为对照组。以汉密尔顿抑郁量表(HAMD)-24减分率评估疗效;采用Spearman相关性分析HAMD-24与自身抗体谱的相关性;Logistic回归分析抑郁症患者预后的影响因素。结果抑郁症患者中血清自身抗体谱阳性率为4.80%(168/3503),其中抗SSA抗体(SSA52和SSA60)阳性率最高4.00%(140/3503)。与对照组相比,观察组血糖(GLU)水平显著降低(4.85±0.87mmol/Lvs5.06±0.78mmol/L),差异具有统计学意义(t=2.900,P<0.01);其他临床指标、治疗前后HAMD-24总分和自杀风险评分差异无统计学意义(t/Z=0.100~2.900,均P>0.05)。与中度抑郁组(8.11%)相比,轻度组(31.82%)和重度组(32.14%)血清抗环胍氨酸肽抗体(抗CCP)的阳性率升高,差异具有统计学意义(χ2=5.510、7.362,均P<0.05)。Spearman分析患者入院时HAMD-24总分与血清抗Jo-1抗体的表达呈负相关(r=-0.15,P<0.05),治疗4周后HAMD-24总分与血清抗SSA52抗体的表达呈正相关(r=0.14,P<0.05)。多因素分析显示,GLU是影响自身抗体谱阳性的抑郁症患者的独立预后因素。结论抑郁症患者血清自身抗体谱抗体表达常见,与抑郁症的进展有一定关系,但未见其与预后的关系。
Abstract:
Objective To explore the expression profiles of serum autoantibodies in patients with depression and their correlation with prognosis. Methods A total of 3 503 patients with depression diagnosed and treated at the Brain Hospital Affiliated to Nan-jing Medical University from July 2018 to March 2024 were enrolled. Serum autoantibody profiles were detected using Western blotting, and clinical data were collected. 115 patients with complete data and positive serum autoantibody profiles were assigned to the observation group, while 86 patients with negative serum autoantibody profiles during the same period were assigned to the control group. The therapeutic efficacy was assessed by the reduction rate of the Hamilton Depression Scale-24 (HAMD-24) score. Spearman correlation analysis was performed to evaluate the relationship between HAMD-24 scores and autoantibody profiles. Logistic regression analysis was used to identify prognostic factors for patients with depression. Results The positive rate of serum autoantibodies in patients with depression was 4.80% (168 / 3 503), with anti-SSA antibodies (SSA52 and SSA60) showing the highest positivity rate of 4.00% (140 / 3 503). Compared with the control group, the blood glucose (GLU)level in the observation group was significantly lower (4.85 ± 0.87 mmol/L vs 5.06 ± 0.78 mmol/L), with a statistically significant difference (t=2.900, P<0.01). Differences in other clinical indicators, pre-and post-treatment HAMD-24 total scores, and suicide risk scores were not statistically significant (t/Z=0.100~2.900, all P >0.05). The positive rate of serum anti-cyclic citrullinated peptide antibody (an-ti-CCP) in patients with mild (31.82%) and severe (32.14%) depression was higher than that in patients with moderate depression (8.11%), with significant differences (χ2=5.510, 7.362, all P< 0.05). Spearman analysis revealed that the HAMD-24 total scores at admission was negatively correlated with the expression of serum anti-Jo-1 antibody (r= -0.15, P < 0.05), while the total HAMD-24 score after 4 weeks of treatment was positively correlated with the expression of serum anti-SSA52 antibody (r = 0.14, P < 0.05). Multivariate analysis indicated that the blood glucose was an independent prognostic factor for depression patients with positive autoantibody profiles. Conclusions The expression of serum autoantibody profiles is common among patients with depression and is associated with the progression of depression, but no relationship with prognosis has been found.
