目的?探讨ABO血型不合异基因造血干细胞移植(allo-HSCT)患者血型转变及造血重建规律,为患者临床输血提供指导性依据。方法选取2019~2023年西安交通大学第一附属医院血液科ABO血型不合allo-HSCT患者37例,其中主要不合组11例、次要不合组17例、主次双向不合组9例,比较三组血型转变时间、造血重建时间以及造血重建周期内红细胞及血小板的输注量差异。结果血型完全转变时间主要不合组162(64,230)天、次要不合组106(63,150)天、主次双向不合组170(145,240)天。与次要不合组相比,主要不合组及主次双向不合组血型完全转变时间明显延长,差异具有统计学意义(Z=16.308、18.139,均P<0.05),主要不合组与主次双向不合组相比差异无统计学意义(Z=0.406,P>0.05)。三组血型初次转变时间、粒系重建时间、巨核系重建时间、红系重建时间、造血重建周期内红细胞及血小板输注量差异无统计学意义(Z=0.521~5.815,均P>0.05)。结论ABO血型不合对allo-HSCT后血型转变存在明显影响,主要不合及主次双向不合患者ABO血型完全转变时间明显延长。
Abstract:
Objective To explore the patterns of blood group conversion and hematopoietic reconstruction in patients undergoing ABO-incompatible allogeneic hematopoietic stem cell transplantation (allo-HSCT), providing guidance for clinical blood transfu-sion management. Methods Thirty-seven patients who undergoing ABO-incompatible allo-HSCT at the Hematology Department of the First Affiliated Hospital of Xi’an Jiaotong University from 2019 to 2023 were enrolled, including 11 in the major ABO-in-compatible group, 17 in the minor ABO-incompatible group, and 9 in the bidirectionally ABO-incompatible group. Differences in time to blood group conversion and hematopoietic reconstruction, and red blood cell and platelet transfusion volumes during the hematopoietic reconstruction period were compared among the three groups. Results The time for complete blood group con-version after ABO-incompatible allogeneic HSCT was 162 (64, 230) days in the major ABO-incompatible group, 106 (63, 150) days in the minor ABO-incompatible group, and 170 (145, 240) days in the bidirectionally ABO-incompatible group. Compared with the minor ABO-incompatible group, the time for complete blood group conversion was significantly longer in the major and bidirectionally ABO-incompatible groups (Z=16.308, 18.139, all P<0.05), while there was no significant difference between the major and bidirectionally ABO-incompatible groups (Z=0.406, P>0.05). There were no significant differences in the time for ini-tial blood group conversion, granulocyte reconstruction time, megakaryocyte reconstruction time, erythroid reconstruction time, or RBC and platelet transfusion volumes during the hematopoietic reconstruction period among the three groups (Z=0.521~5.815, all P>0.05). Conclusions ABO incompatibility significantly affects blood group conversion after allo-HSCT, with a significantly prolonged time to complete blood group conversion in patients with major and bidirectionally ABO-incompatible allogeneic HSCT.
