摘要:
目的?依据美国临床和实验室标准协分(CLSI)EP7-A3文件评价10g/L血红蛋白、475μmol/L结合胆红素、684μmol/L游离胆红素和16.94mmol/L甘油三酯对高效液相色谱串联质谱法(HPLC-MS/MS)检测血浆3种脂溶性维生素[VA、25(OH)D、α-VE]的干扰。方法收集2025年1~2月北京航天总医院临床检测剩余血浆作为基础样本,将基础样本和干扰物浓缩液混合配制成干扰样本,基础样本和稀释液混合配制成对照样本,采用配对差异实验确定4种物质对3种脂溶性维生素是否存在干扰作用,采用剂量效应实验明确不同浓度的干扰物带来的结果差异。结果?配对差异实验中,溶血、结合胆红素、游离胆红素对VA、25(OH)D、α-VE的检测结果不存在干扰。甘油三酯对VA、25(OH)D的检测没有干扰,对α-VE存在正干扰,干扰样本与对照样本的相对偏差分别为140.15%、50.00%。剂量效应实验结果显示当甘油三酯浓度为2.83、5.66、8.48和11.31mmol/L时,低浓度α-VE中干扰样本与对照样本的相对偏差分别为32.30%、57.61%、9.04%和130.48%,高浓度α-VE中相对偏差分别为23.45%、40.78%、47.78%和64.37%。结论溶血、结合胆红素、游离胆红素对VA、25(OH)D、α-VE的检测结果不存在干扰,甘油三酯对α-VE的检测存在正干扰,随着甘油三酯浓度增加,干扰效应增强,需引起临床实验室的注意。
Abstract:
Objective To evaluate the interference of 10 g/L hemoglobin, 475 μmol/L conjugated bilirubin, 684 μmol/L free bilirubin and 16.94 mmol/L triglyceride on the detection of three fat-soluble vitamins [VA, 25(OH)D, α-VE] in plasma by high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) according to the Clinical and Laboratory Standards Institute (CLST) EP7-A3 document. Methods The remaining plasma from clinical tests at Beijing Aerospace General Hospital from January to February 2025 was collected as the basic sample. Interference samples were prepared by mixing basic samples with concentrated interferents, while control samples were prepared by mixing basic samples with diluents. Paired difference experiments determined whether the four substances interfered with the detection of the three fat-soluble vitamins, and dose-response experiments clarified the differences in results caused by varying concentrations of interferents. Results In paired difference experiments, hemolysis, conjugated bilirubin and free bilirubin did not interfere with the detection of VA, 25(OH)D, or α-VE. Triglycerides did not interference with the detection of VA or 25(OH)D, but positively interfered with α-VE, yielding relative deviations of 140.15% and 50.00% between the interference samples and the control samples, respectively. The results of the dose-effect experiment showed that at triglyceride concentration of 2.83, 5.66, 8.48 and 11.31mmol/L, the relative deviations between the interference samples and the control samples in the low-concentration α-VE were 32.30%, 57.61%, 9.04% and 130.48%, respectively. The relative deviations in high-concentration α-VE were 23.45%, 40.78%, 47.78% and 64.37%. Conclusions Hemolysis, conjugated bilirubin, and free bilirubin did not interfere with the detection of VA, 25(OH)D or α-VE. Triglycerides positively interfered with the detection of α-VE, with the interference effect increasing as the concentration of triglycerides rose. This requires attention from clinical laboratories.
