摘要:
目的 探讨直接抗人球蛋白试验(DAT)阳性患者的交叉配血方法。方法 采用DAT法筛查红细胞表面的不规则抗体,微柱凝胶法进行血型鉴定,凝聚胺法和抗人球蛋白法交叉配血。结果 共计5例DAT阳性的疑难配血患者,其血型分别为A-DCcEe,O-DCCee,O-DCcee,A-DCCee和A-DCcee。选择相应血型抗原阴性的供者血样进行交叉配血,凝集强度均弱于自身对照,建议观察输注洗涤红细胞,患者未发生不良反应。结论 对于DAT阳性的患者,可配合性输注相应血型抗原阴性的洗涤红细胞。
Abstract:
Objective To investigate the appropriate cross-matching methods for the direct antiglobulin test(DAT)positive patients.Methods The DAT was used to screen the irregular antibodieson the surface of erythrocytes.The microcolumn gel test was used for blood identification.The antiglobulin test and manual polybrene test were used for cross-matching.Results A total of five DAT positive cases were collected in study.The blood groups of each case were detected,including A-DCcEe,O-DCCee,O-DCcee,A-DCCee and A-DCcee.The donor samples with corresponding blood-antigen negative were chosen for cross-matching,and the agglutination intensity of each cross-matching tube was weaker than the control.Thetransfusion of washed red blood cells was suggested.Finally,each transfusion was safe and efficient.Conclusion The infusion of the washed red blood cells with corresponding blood-antigen negative could efficientlyensure the safety of blood transfusion.
