目的探讨血清促红细胞生成素(erythropoietin，EPO)、红细胞(RBC)相关参数水平，为临床应用重组人红细胞生成素(recombinant human erythrop ietin，rHuEPO)治疗慢性呼吸衰竭贫血检测提供理论数据。方法收集25例慢性呼吸衰竭患者的血清，应用酶联免疫法检测EPO水平和RBC相关参数水平，并将其与20例正常对照进行比较，以直线相关分析EPO水平和RBC相关参数之间的关系。结果慢性呼吸衰竭贫血患者血清中EPO水平为99.70±28.90 mIU/ml，显著高于正常对照组的21.5±5.26 mIU/ml，二者比较差异有统计学显著性意义(t=2.67，P<0.01)。两组Hb，HCT水平差异有统计学显著性意义(t=2.52，2.23，均P<0.01)，RBC两组差异有统计学意义(t=2.12，P<0.05)。患者的血清EPO水平与RBC，Hb，HCT值之间呈负相关关系(均P<0.01)。结论呼吸衰竭贫血患者血清EPO水平可反应性增高，但个体间有差异，应根据各种检测数据制定治疗方案，动态检测血清EPO水平，对呼吸衰竭贫血的治疗以及预后判断有重要的临床参考价值。
Abstract:
ObjectiveIn order to provide a theoretical basis for clinical use of Recom2binant Human Erythrop ietin(rHuEPO) to treat chronic respiratory failure，investigate the relationship between serum erythropoietin (EPO) and red blood cell count (RBC) levels in patients with anemia of chronic respiratory failure．MethodsThe level of serum EPO levels and RBC-related parameters in tweenty-five patients with chronic respiratory failure was measured by enzyme-linked immunoassay (ELISA)．The results were compared to tweenty normal controls．The relationship between EPO levels and RBC-related parameters was analyzed in linear correlation analysis．ResultsThe chronic respiratory failure patients with anemia serum EPO level for 99.70±28.90 mIU/ml was significantly higher than that of normal control group (21.5±5.26 mIU/ml)．Two groups of Hb，HCT levels differed significantly (t=2.52，2.23，all P<0.01)．There were differences between the two groups (t=2.12，P<0.05) for RBC．There was a significant difference in the serum EPO levels(t=2.67，P<0.01) between the control group and patients with anemia of chronic respiratory failure．The increases extent of EPO had some inconsistencies with severity of anemia，had negative correlation with relevant parameter values．ConclusionEPO levels could be increase in respiratory failure patients with anemia．There were differences between individuals．The principle of individualized treatment should be noted．The rHuEPO treatment measures should be selected vary to individual．Dynamic detection of serum EPO levels has important clinical value on the treatment and prognosis of respiratory failure with anemia．
