摘要:
目的 分析陕西地区结核分枝杆菌(MTB)对利福平(RFP)耐药性及rpoB基因突变情况。方法 收集2013~2015年陕西省结核病防治研究所和陕西省结核病防治院怀疑为肺结核的患者标本3 739例,采用利福平耐药实时荧光定量核酸扩增检测技术(Xpert MTB/RIF)对其进行RFP耐药性及rpoB基因突变检测,选取检测为MTB阳性并诊断为结核病患者的结果1 506例进行RFP耐药性及rpoB基因突变情况分析,采用SPSS18.0软件的卡方检验进行统计学分析P<0.05,差异有统计学意义。结果 2013 ~2015年RFP耐药率分别为17.0%,14.1%和12.1%,其中2013年RFP耐药率显著高于2015年,耐药率差异有统计学意义(χ2=4.843,P=0.028); 三年利福平总耐药率为14.4%,其中肺内组RFP总耐药率15.0%,显著高于肺外组8.1%,差异有统计学意义(χ2=4.713,P=0.03); 四个年龄组RFP耐药率差异有统计学意义(χ2=13.684,P=0.03),其中41~60岁年龄组耐药率显著高于其他年龄组(P<0.05)。利福平耐药rpoB基因突变有一定区域性,突变区域主要为529~533区域并存在多位点突变的现象。结论 陕西地区结核分枝杆菌对RFP耐药水平较高,应加强陕西省的结核病耐药监测。
Abstract:
Objective To analyze rifampin(RFP)resistance of Mycobacterium tuberculosis(MTB)and the mutation of rpoB gene in Shaanxiarea.Methods Collected 3 739 specimens of suspected pulmonary tuberculosis patients in Shaanxi Provincial Institute for Tuberculosis Control and Prevention and Hospital of Shaanxi for Tuberculosis Prevention and Treatment from 2013 to 2015.And all the specimens were detected by the RFP resistance real-time fluorescence quantitative PCR(Xpert MTB/RIF)and selected 1 506 casesof patients with tuberculosis for RFP resistance and rpoB gene mutation analysis.χ2 test was used for statistical analysis(P<0.05)difference wasstatistically significant.Results The resistance rates for RFP from 2013 to 2015 were 17.0%,14.1% and 12.1%.The rate of RFP resistant in 2013 was significantly higher than the rate in 2015 and the difference wasstatistically significant(χ2=4.843,P=0.028).The average rate of RFPresistant was 14.4% in three years.The RFP resistance rate of lung and sputum specimens(15.0%)was significantly higher than that of the specimens outside of the lung(8.1%),and the difference was statistically significant(χ2=4.713,P=0.03).There was no significant difference between male and femalegroups(χ2=1.048,P=0.306).The RFP resistance rate of age from 41 to60 was significantly higher than other age groups,and the difference was statistically significant(χ2=13.684,P=0.03).RFP resistant rpoB gene mutations were regional and region 529~533 had mutation.There were also a number of point mutations.Conclusion The RFP resistance level to Mycobacterium tuberculosis was high in Shaanxi area and the surveillance of drug resistance in tuberculosis should be strengthen in Shaanxi province.
