摘要:
目的 比较在梅毒筛查中甲苯胺红不加热血清试验(TRUST) 改良方法与传统方法的检测性能。方法 对TRUST高滴度的梅毒血清 进行一系列稀释,分别用改良方法与传统方法进行测试,计算出一系列浓度下每种方法的阳 性率。比较两种检测方法的C50,C5~C95区间,以及不精密度曲线,并对两 种方法的一致性,以及诊断性能进行比较。结果 改良方法的C5 0小于传统方法,改良方法的C5~C95区间较传统方法窄,改良方法的不精密度曲 线较传统方法陡峭,两种方法一致性程度的95%的可信区间为73.4%~95.8%。改良方法灵 敏度、特异度、阳性预测值、阴性预测值和诊断符合率分别为64.29%,99.1%,85.71%, 97.05%和96.39%,传统方法灵敏度、特异度、阳性预测值、阴性预测值、诊断符合率分别 为53.75%,98.49%,75%,96.18%和95%。改良方法的灵敏度、阳性预测值高于传统方法 ,差异有统计学意义(χ2=8.25,10.03,P均<0.05)。改良方法的特异度、阴 性预测值、诊断符合率均略高于传统方法,差异无统计学意义(χ2=2.39,3.45,4 .03,P均>0.05)。结论 改良方法的精密度、灵敏度和特异度较 传统方法高,可以借助全自动酶免仪加样,适用大批量样本的检测,改良方法可代替传统方 法用于梅毒筛查。
Abstract:
Objective To compare the detection performance of the modified serum test (TRUST) method and the traditional method in syphilis screening.Methods A series of TRUST high titer syphilis serum was diluted, and the positive rate of each method was calculated by usin g the improved method and the traditional method.Comparison of two detection m ethods of C50,C5 ~ C95 interval,as well as the accuracy of the d ensity curve,and the consistency of the two methods were compared,and diagnost ic performance were compared.Results The improved method of C50 was less than the traditional method of C50,and the improve d method of C5 ~ C95 range was narrower than the traditional method,com pared with the traditional method.The improved method of the non precision dens ity curve was steeper than the traditional method,and the two confidence interv al of the consistency degree of the 95% methods was 73.4% to 95.8%.The diagno stic sensitivity (SEN),diagnostic specificity (SPE),positive predictive value (PPV),negative predictive value (NPV) and diagnostic efficiency(DF) of the impr oved method were 64.29%,99.1%,85.71%,97.05% and 96.39%,respectively.Th e SEN,SPE,PPV,NPV and DF of the traditional methods were 3.75%,98.49%,75% ,96.18% and 95%,respectively.The improved method was superior to the traditi onal methods in the two aspects of SEN and PPV (χ2=8.25,10.03,all P <0.05),with the statistically significant difference.The improved method wa s slightly higher than the traditional method in SPE,NPV and DF (χ2=2.3 9,3.45,4.03,all P>0.05),with the no statistically significant differ ence.Conclusion The precision,diagnostic sensitivity an d diagnostic specificity of the improved method was higher than that of the trad itional method,and it can be applied to the detection of large batch samples wi th the aid of the full automatic enzyme immunoassay instrument.The improved met hod can be used to replace the traditional method for syphilis screening.
