摘要:
目的 探讨五羟色胺转运体基因启动子区(5-hydroxytryptamine transportergene-linked polymorphic region,5-HTTLPR)基因多态性与人格特征对难治性抑郁症(treatment resistant depression, TRD)易感性的影响。方法 运用聚合酶链反应-限制性片段长度多态(PCR-RFLP)测定180例难治性抑郁症患者(研究组)和180例非难治性抑郁症(Non resistant depression, nTRD)患者(对照组)的5-HTTLPR基因多态性。应用汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)评估两组患者严重程度,明尼苏达多项人格测验(Minnesota Multiphasic Personality Inventory,MMPI)分析两组患者的人格特征,分析基因型与人格特征的交互作用对难治性抑郁症易感性的影响。结果 难治性抑郁症患者组与非难治性抑郁症组基因型分布差异有统计学意义(χ2=6.53,P=0.044),其中难治性抑郁症患者组SS基因型频率高于非难治性组,两组患者等位基因差异有统计学意义(χ2=6.74,P=0.012)。难治性抑郁症患者MMPI量表疑病因子分(t=3.94,P=0.001)、抑郁因子分(t=3.87,P=0.004)、社会内向分(t=3.87,P=0.003)均高于非难治性组,差异均有统计学意义。Logistic回归分析显示,SS基因型与疑病、抑郁、内向因子的交互作用进入难治性抑郁的回归方程(OR=2.574,95%CI:1.109~4.937; OR=1.388,95%CI:1.048~4.811; OR=2.724,95%CI:1.134~5.873)。结论 SS基因型可能是难治性抑郁症的易感因素,难治性抑郁症患者存在更多的人格问题,基因型与人格特征的交互作用更易引起难治性抑郁症的发生。
