摘要:
目的 探讨血浆游离DNA中Septin9 甲基化(mSEPT9) 检测在结直肠癌中的诊断意义。方法 收集2019 年6 ~ 11月在重庆医科大学附属第二医院接受肠镜检查的患者共100 例,包括确诊为结直肠癌的患者45 例,非结直肠癌对照组55 例,对其进行mSEPT9 检测、大便隐血试验(FOBT) 和癌胚蛋白(CEA)检测,评价其在结直肠癌中的应用价值。结果 ① mSEPT9 筛查结直肠癌的敏感度为82.22%(95%CI:70.60% ~ 93.83%), 明显高于CEA(42.2%, 95%CI:27.22%~57.23%), 特异度为89.09%(95%CI:80.58%~97.60%), 高于FOBT(75%, 95%CI:60.98%~89.02%)。②结直肠癌组mSEPT9 阳性率为82.22%(37/45), 非结直肠癌对照组mSEPT9 阳性率为10.91%(6/55),结直肠癌组和非结直肠癌组相比,mSEPT9 检测结果差异有统计学意义(χ2=4.917, P<0.05)。③ mSEPT9 检测结果与患者性别、年龄、病例分化程度无相关性(χ2=0.020, 1.142, 1.649, P<0.05)。结论 mSEPT9 检测在结直肠癌诊断中有较高的敏感度和特异度,并且具有无创、采样方便、方法稳定等优势,可以作为临床诊断结直肠癌的辅助手段。
