摘要:
目的 探讨原发性高血压并发肾病患者心率变异性与尿微量清蛋白 / 尿肌酐比值(UACR)的相关性,分析UACR,心率变异性参数在原发性高血压肾功能损害的诊断价值。方法 选择重庆市长寿区中医院 2017 年 2 月 ~2019年 6 月收治的 106 例原发性高血压患者,根据是否并发肾病将其分为原发性高血压组(57 例)和并发肾病组(49 例),另选择 50 例健康志愿者为对照组。均检测 UACR,行动态心电图检测,记录心率变异性参数 [ 总体标准差 (SDNN) ,均值标准差 (SDANN) ,差值均方根(rrMSSD),差值 >50ms 百分比 (PNN50),低频功率 (LF),高频功率(HF)和 LF/HF]。Pearson 相关性分析 UACR 与心率变异性参数的相关性。受试者工作特征曲线(ROC)分析 UACR,心率变异性参数诊断原发性高血压并发肾病的价值。结果 SDNN,LF 和 LF/HF 在对照组、原发性高血压组、并发肾病组依次降低(F=37.420,17.342 和 23.616,均 P < 0.05),UACR 依次升高(F=78.609,P < 0.05),各组间差异显著(t=9.265~16.352,P 均< 0.05),SDANN,rMSSD,PNN50 和 HF 在各组间差异无统计学意义(F=0.017,0.175 和 0.078,0.471,均 P >0.05)。并发肾病组 UACR 与 SDNN,LF 和 LF/HF 呈负相关(r = -0.513、- 0.538 和 - 0.612,均 P < 0.05),与SDANN,rMSSD,PNN50 和 HF 无关(r=0.115,0.171,0.182 和 0.162,均 P > 0.05)。ROC 分析结果显示 UACR,SDNN,LF,LF/HF联合 UACR,SDNN,LF 与 LF/HF(以下简称联合诊断)诊断原发性高血压并发肾病的曲线下面积(AUC)分别为 0.731,0.749,0.683,0.772 和 0.885,联合诊断高于单独 UACR,SDNN,LF 和 LF/HF 诊断价值(Z=2.156,2.035,3.021 和2.013,均 P < 0.05)。结论 UACR 与原发性高血压并发肾病患者心率变异性参数 SDNN,LF 和 LF/HF 存在显著相关性,UACR,SDNN,LF 和 LF/HF 可用作预测原发性高血压患者肾功能损伤的敏感指标。
