摘要:
目的 研究单采献血者白细胞介素-28b(IL-28b)基因多态性与隐匿性乙型肝炎病毒(HBV)感染(OBI)相关性。方法 选择2016年1月~2019年12月山东省血液中心成功捐献血小板HbsAg(-)、核酸检测(NAT)无反应的献血者1 158例为研究对象,提取献血者血液DNA,采用直接测序法进行IL-28b基因分型,cobas HBV定量核酸检测试剂盒检测血液HBV-DNA载量,Logistic回归分析IL-28b基因rs8099917位点、rs12979860位点多态性与OBI的关系。结果 与非OBI比较,OBI献血者IL-28b基因rs8099917位点G等位基因频率、TG+GG基因型频率显著降低,T等位基因频率、TT基因型频率显著增加(χ2=22.137,22.163,均P<0.01);rs12979860位点T等位基因频率、CT+TT基因型频率显著降低,C等位基因频率、CC基因型频率显著增加(χ2=16.378,19.091,均P<0.01)。OBI献血者rs8099917位点,与TT基因型比较,TG+GG基因型患者HBV-DNA载量显著降低(t=5.257,P<0.01);rs12979860位点,与CC基因型比较,CT+TT基因患者HBV-DNA载量显著降低(t=17.398,P<0.01),以上比较差异均有统计学意义。Logistic回归分析结果显示,rs8099917位点,与TT基因型比较,TG+GG基因型可显著降低OBI发生风险(95% CI:0.288~0.843,P<0.05) ;rs12979860位点,与CC基因型比较,CT+TT基因型可显著降低OBI发生风险(95% CI :0.207~0.761,P<0.05)。结论 单采献血者OBI可能与IL-28b基因rs8099917位点、rs12979860位点多态性有关。
Abstract:
Objective To study the correlation between interleukin-28b (IL-28b) gene polymorphism and occult hepatitis B virus(HBV) infection (OBI) in platelet donors. Methods From January 2016 to December 2019, 1 158 platelet donors fromshandong Blood Center who successfully donated platelets with HBsAg (-) and had no response to nucleic acid test (NAT) wereselected as the study subjects, DNA was extracted from blood donors, IL-28b was genotyped by direct sequencing, cobas HBVquantitative nucleic acid detection kit was used to detect HBV-DNA load in blood, and Logistic regression was used to analyzethe relationships between the polymorphisms of rs8099917, rs12979860 loci on IL-28b gene and OBI. Results Compared withnon OBI, the G allele frequency and TG + GG genotype frequency at rs8099917 locus on IL-28b gene in OBI donors weresignificantly lower, while the T allele frequency and TT genotype frequency were significantly higher (χ2=22.137, 22.163, allP<0.01). T allele frequency and CT + TT genotype frequency of rs12979860 were significantly lower, while C allele frequencyand CC genotype frequency were significantly higher (χ2=16.378, 19.091, all P<0.01). At rs8099917 locus of OBI donors, HBVDNAload of TG + GG genotype was significantly lower than that of TT genotype (t=5.257, P<0.01). At rs12979860 locus, HBVDNAload of CT + TT genotype was significantly lower than that of CC genotype(t=17.398, P<0.01), and the above differenceswere statistically significant. Logistic regression analysis showed that, at rs8099917 locus, compared with TT genotype, TG + GGgenotype could significantly reduced the risk of OBI (95% CI:0.288~0.843, P<0.05). At rs12979860 locus, compared with CCgenotype, compared with CC genotype, CT + TT genotype could significantly reduced the risk of OBI (95% CI: 0.207~0.761, P<0.05). Conclusion  OBI of platelet donors may be related to the polymorphisms of rs8099917 andrs12979860 loci on IL-28b gene.
