摘要:
目的 分析系统性红斑狼疮(systemic lupus erythematosus ,SLE)病人血清中IL-1 家族细胞因子、拮抗剂与可溶性受体含量变化与临床价值。方法 选取泸州市中医医院于2018 年6 月~ 2020 年6 月期间收治的60 例SLE 病人为观察组,同期60 例健康体检人员为对照组,另基于SLE 疾病活动指数评分(SLEDAI),划分观察组为活动组和稳定组。对比上述三组IL-1 家族细胞因子、拮抗剂、可溶性受体含量,并对SLE 患者细胞因子水平与免疫指标ds-DNA,补体C3,C4,C1q 的相关性进行分析。结果 观察组与对照组比较,细胞因子IL-33[1.5(0~2.90) vs 0.2(0~1.46)]pg/ml,IL-18
[329.2(271.5~386.2) vs 430.0(314.6~587.8)]pg/ml,拮抗剂IL-18BP
[6 455(5 021~8 357) vs 9 863(6 982~15 746)]pg/ml,IL-1Ra[173.5(120.3~213.8) vs 226.9(129.9~ 478.2)]pg/ml 和可溶性受体IL-1R4[1 004(7 356~12 553) vs 15 870(12 336 ~23 569)]pg/ml 差异均有统计学意义(t=7.446,8.513,10.256,5.864,12.257,均P<0.05)。SLE 患者细胞因子IL-18,free IL-18,IL-1R2,IL-1R4 与多项自身抗体或补体间存在相关性(r=-0.328~0.412,均P<0.05)。SLE 活动组患者IL-18,free IL-18,IL-1R4 细胞因子水平与稳定组患者间差异有统计学意义(均P<0.05)。结论 IL-18 和IL-1R4 可能为预测SLE 患者疾病活动度的候选生物标志物。
Abstract:
Objective To analyze the changes and clinical value of IL-1 family cytokines, antagonists and soluble receptors in systemic lupus erythematosus(SLE) patients. Methods Selected a total of 60 SLE patients treated in the Luzhou Traditional Chinese Medicine Hospital from June 2018 to June 2020 as the observation group , while subjects in the control group were 60 healthy physical examination personnel in the same period. In addition, based on SLEDAI , the observation group was divided into the active group and the inactive group. The contents of IL-1 family cytokines, antagonists and soluble receptors in the above three groups were compared, and the correlation between the levels of cytokines in SLE patients and immune indexes ds-DNA, complement C3, C4 and C1q was analyzed. Results  There were significant differences in cytokines IL-33[1.5(0~2.90) vs 0.2(0~1.46)]pg/ml, IL-18

There was a correlation between cytokines IL-18, freeIL-18, IL-1R2, IL-1R4 and multiple autoantibodies or complements in SLE patients (r=-0.328~0.412,all P < 0.05). The levels of IL-18, free IL-18 and IL-1R4 cytokines in SLE active group were significantly higher than those in stable group (all P < 0.05). Conclusion IL-18 and IL-1R4 may be candidate biomarkers for predicting disease activity in SLE patients.
