摘要:
目的 研究孕妇血清收集素(Collectrin)和脑啡肽酶(neprilysin,NEP)表达水平与子痫前期(preeclampsia,PE)的相关性。方法 选取2020 年1 月~ 2021 年1 月湖北理工学院附属妇幼保健院收治的87 例PE 患者为PE 组,根据病情严重程度分为轻度子痫前期组(n=38)和重度子痫前期组(n=49),另选取同期42 例健康妊娠妇女为对照组。酶联免疫吸附试验(enzyme linke immunosorbent assay,ELISA)检测血清Collectrin 和NEP 水平,Pearson/Spearman 相关性分析PE患者血清Collectrin 和NEP水平与血压的相关性,ROC曲线分析血清Collectrin 和NEP水平对PE的预测价值。结果 与对照组比较,PE 组NEP[102.72(97.76,105.84) pg/ml vs 71.20(62.82,87.48)pg/ml] 水平升高,Collectrin(163.82±66.00pg/ml vs 241.39±67.31 pg/ml)水平降低,差异有统计学意义(Z=6.172,t =8.499,均P < 0.001)。与轻度子痫前期组比较,重度子痫前期组NEP[ 84.00(68.10, 95.33) pg/ml vs 63.71(60.61, 80.46)pg/ml] 水平升高,Collectrin(204.58±62.96pg/ml vs 269.93±56.19 pg/ml) 水平降低, 差异均有统计学意义(Z=4.433,t=5.104, 均P< 0.05)。Pearson/Spearman 相关性分析显示,PE 患者收缩压、舒张压与血清Collectrin 水平呈负相关(r=-0.656,-0.748,均P < 0.001),与血清NEP 水平呈正相关(r=0.638,0.694,均P < 0.001)。ROC 曲线显示,Collectrin,NEP 和Collectrin + NEP 预测PE 的曲线下面积分别为0.843,0.804 和0.914,敏感度分别为74.71%,60.92% 和81.61%,特异度分别为92.86%,90.48% 和92.86%。Collectrin + NEP 预测PE 的曲线下面积大于Collectrin 和NEP 单独预测,差异均有统计学意义(Z=2.984,3.540,均P < 0.05)。结论 PE 患者血清Collectrin 水平降低,NEP 水平升高,与血压水平密切相关,可作为PE 预测指标。
Abstract:
Objective To investigate the correlation between the expression levels of serum Collectrin and Neprilysin (NEP) in pregnant women and pre-eclampsia. Methods 87 patients with PE admitted to the Maternal and Child Health Hospital Affiliated to Hubei Institute of Technology from January 2020 to January 2021 were selected as the PE group, and were divided into mild pre-eclampsia group (n=38) and severe pre-eclampsia group (n=49) according to the severity of the disease, and another 42 healthy pregnant women in the same period were selected as the control group. ELISA was used to detect serum Collectrin and NEP levels, Pearson/Spearman correlation was used to analyze the correlation between serum Collectrin and NEP levels and blood pressure in PE patients, and ROC curve was used to analyze the predictive value of serum Collectrin and NEP levels on PE. Results Compared to the control group, the PE group had increased levels of NEP [102.72 (97.76, 105.84) pg/ml vs 71.20 (62.82, 87.48) pg/ml] and decreased levels of Collectrin (163.82±66.00 pg/ml vs 241.39±67.31 pg/ml), with differences statistically significant (Z=6.172,t =8.499,all P < 0.001). Compared to the mild pre-eclampsia group, the severe preeclampsia group had increased levels of NEP [84.00 (68.10, 95.33) pg/ml vs 63.71 (60.61, 80.46) pg/ml] and decreased levels of Collectrin (204.58±62.96 pg/ml vs 269.93±56.19 pg/ml ), with statistically significant differences(Z=4.433,t=5.104,all P < 0.05).Pearson/Spearman correlation analysis showed that systolic and diastolic blood pressure in PE patients were negatively correlated with serum Collectrin levels (r = -0.656, -0.748, all P < 0.001) and positively correlated with serum NEP levels (r = 0.638, 0.694, all P < 0.001).The ROC curves showed that the area under the curve for PE prediction by Collectrin, NEP and Collectrin + NEP were 0.843, 0.804 and 0.914, respectively, with sensitivities were 74.71%, 60.92% and 81.61%, and the specificity were 92.86%, 90.48% and 92.86%, respectively. The area under the curve for PE predicted by Collectrin + NEP was greater than that predicted by Collectrin and NEP alone, the differences were statistically significant (Z = 2.984, 3.540, all P < 0.05).Conclusion Reduced serum Collectrin levels and increased NEP levels in PE patients were closely correlated with blood pressure levels and can be used as predictors of PE.
