摘要:
目的 探讨尿肾脏损伤分子-1(KIM-1)和嗜中性粒细胞明胶酶相关脂质转运蛋白(NGAL)在糖尿病肾病(DN)早期诊断中的应用。方法 选择1型和2型糖尿病患者132例,依据尿蛋白/血肌酐比(ACR)和血清肌酐水平分为正常蛋白尿组(45例)、微量蛋白尿组(36例)、临床清蛋白尿组(30例)、肾功能衰竭组(21例),以65例健康体检者为对照组,酶联免疫吸附法(ELISA)测定尿KIM-1和NGAL含量。结果 随着DN的进展,尿KIM-1/Ucr和NGAL/Ucr水平逐渐增高,且各组间差异有统计学显著性意义(P<0.05); 尿KIM-1/Ucr水平与ACR呈正相关(r=0.822,P<0.01),尿NGAL/Ucr水平与ACR呈正相关(r=0.842,P<0.01)。结论 尿KIM-1/Ucr和NGAL/Ucr水平不但可以更早预测DN的发生,还可以监测DN的进展。
Abstract:
Objective To investigate the value of KIM-1 and NGAL in predicting the early diabetes nephropathy(DN).Methods 132 cases of type 1 and type 2 diabetic patients were recruited in thisstudy,patients were divided into four groups based on the urine albumin/creatinine and serum creatinine levels,as follows:normal albuminuria group(45 cases),microalbuminuria group(36 cases),clinical albuminuria group(30 cases),renal failure group(21 cases),65 healthy subjects were recruited as control group.Urinary KIM-1 and NGAL levels were measured by enzyme linked immunosorbent assay(ELISA).Results With the progress of DN,urinary KIM-1 and NGAL levels gradually increased(P<0.05),the urinary KIM-1/Ucrlevel was positively correlated with ACR(r=0.822,P<0.01),and the urinary NGAL/Ucr was positively correlated with ACR(r=0.842,P<0.01).Conclusion The urinary KIM-1 and NGAL level can predict the occurrence of early DN,and also monitor the progression of DN.
