摘要:
目的 通过对尿毒症患者血液透析前后血清蛋白成分变化情况的分析,探讨血液透析对机体炎症状态及免疫功能的影响。方法 选取2013年10月~2014年5月经肾病内科确诊的尿毒症患者75例为观察组,同期15名健康体检者为对照组,将观察组首次透析前、首次透析后、规律透析治疗一个月后以及对照组的血清蛋白电泳图谱相互进行比较。结果 与对照组相比,观察组首次透析前及首次透析后清蛋白水平降低,α1和α2球蛋白水平升高,差异有统计学显著性意义(P<0.001)。首次透析后α1球蛋白水平降低,差异有统计学意义(P<0.01)。透析治疗一个月后清蛋白与首次透析前的差异无统计学意义(P>0.05),而α1和α2球蛋白较透析前明显降低,差异有统计学意义(P<0.001),清蛋白低于对照组差异有统计学意义(P<0.001),γ球蛋白与对照组、首次透析前及首次透析后比较水平升高,差异均有统计学意义(P<0.001或P<0.01)。结论 规律有效的血液透析可以改善尿毒症患者机体的炎症状态增强机体免疫功能。
Abstract:
Objective To study the influence of hemodialysis on inflammatory state and immune function by analyzing the change of serum protein components in uremic patients before and after hemodialysis.Methods 75 cases of uremic patients confirmed by the Nephrology from October 2013 to May 2014 were selected as the observation group,and 15 healthy volunteers at the same time as the control group.Then the serum protein electrophoresis pattern of observation group beford the first hemodialysis,after the first hemodialysis,after one month's treatment and control group were compared with each other.Results In the observation group before and after the first hemodialysis,the ALB levels were lower,α1 and α2 globulinlevels were higher than those in control group.There was a statistically significant between the observation group before and after the first hemodialysis andcontrol group(P<0.001).After the first hemodialysis,there were differences in α1 globulin levels compared with before the first hemodialysis(P<0.01).ALB was no significant difference after one month's hemodialysis comparedwith before the first hemodialysis,and α1,α2 globulin were significantly reduced and the difference was statistically significant(P<0.001),ALB was lower than the control group and was statistically significant(P<0.001).After one month's hemodialysis,the levels of γ globulin were higher than thosein control group,before the first hemodialysis and after the first hemodialysis.There were significant differences(P<0.001 or P<0.01).Conclusion Regular and effective hemodialysis can improve inflammatory state and immune function of uremic patients.
