摘要:
目的探讨创伤性失血性休克(traumatic hemorrhagic shock,THS)中血浆乳酸脱氢酶(lactate dehydrogenase,LDH)动态变化及其临床价值。方法选择无特定病原体成年大鼠19只,通过急性失血方式建立收缩压下降曲线,形成THS动物模型,测定收缩压基础值(T1),记录收缩压降至基础值3/4(T2),基础值1/2(T3),基础值2/5(T4),基础值1/5(T5),10 mmHg(T6)时失血量,采用酶联免疫吸附分析检测各时间点血浆LDH含量水平。结果T2~T3及T4~T5时间段组内差异有统计学意义(P<0.05),T3~T4及T5~T6时间段组内差异无统计学意义(P>0.05),T2~T3时间段组内LDH含量水平显著性升高,T4~T5时间段组内LDH含量水平显著性下降。结论选择收缩压下降曲线建立THS模型评估失血量变化,结合血浆LDH含量水平动态监测,可对THS过程中组织细胞损伤程度进行有效评估,LDH含量水平可作为判断组织细胞损伤反应的血清学指标。
Abstract:
ObjectiveTo explore the dynamic changes in the plasma levels of lactate dehydrogenase(LDH) with traumatic hemorrhagic shock(THS),MethodsThe model of the THS with gradient decerease in the systolic blood pressure was eatablished in 19 rats without specific pathogen.The arterial blood during the different phase of shock,i.e.when the systolic pressures were 3/4,2/1,5/2 of the basis systolic blood pressure,5/5 of the basic systolic blood pressure and 10 mmHg,was collected and LDH levels in the plasma were determined by the double antibody sandwich method of enzyme-labeled immunosorbentassay(ELISA) and analyzed.ResultsThere were significant differences in plasma levels of LDH between the rats in T2~T3 and T4~T5 period of time (P<0.05),and there was no significant differences in plasma levels of LDH between the T3~T4 and T5~T6 period of time(P>0.05),and LDH levels in T2~T3 period of time were significant increased with the extension of time,with LDH levels in T4~T5 period of time were significant falling with the extension of time.ConclusionThe animal model of THS is related to the systolic pressure during the shock,may be helpful to assessment changes in blood loss.The dynamic observation of the plasma levels of LDH which is a good index of the damage to the tissue cells,may be helpful to assessment of the THS is related to the degree of damage to the tissue cells.LDH levels can be serological indicators used as tissue cells damage reaction.
