摘要:
目的对南京中医药大学附属医院院内分离出来的鲍曼不动杆菌进行各类β-内酰胺酶基因的检测,探讨这些基因的分布特点,研究鲍曼不动杆菌的耐药机制。方法收集2014年1月~12月期间医院内分离的200株鲍曼不动杆菌,最低抑菌浓度(MIC)法测定其对抗生素的敏感性;PCR法检测A类β-内酰胺酶基因SHV,GES,TEM;B类β-内酰胺酶基因VIM,IMP;C类β-内酰胺酶基因AmpC和D类β-内酰胺酶基因OXA-51,OXA-23,OXA-24,OXA-58在菌株中的分布。结果200株鲍曼不动杆菌中116株为多重耐药株,其对常用抗生素的耐药率超过70%;其中52株携带TEM,携带率为26.0%,135株携带SHV,携带率为67.0%,7株检出GES,携带率为3.5%;170株携带VIM-1,携带率为85.0%,12株检出IMP-1,携带率为6.0%;150株携带AmpC,携带率为75.0%;190株携带OXA-51,携带率为95.0%,165株携带OXA-23,携带率为82.5%,2株携带OXA-58,携带率为1.0%,所有菌株未检出OXA-24。结论携带β-内酰胺酶基因是鲍曼不动杆菌的主要耐药机制之一,携带率最高的基因为OXA-51,OXA-23,AmpC,VIM-1和SHV,携带率均超过60%,说明鲍曼不动杆菌的耐药是A,B,C,D四类β-内酰胺酶基因协同作用的结果。
Abstract:
ObjectiveTo study the β-lactamase genes in Acinetobacter baumannii (Ab) acquired in Affiliated Hospital of Nanjing University of TCM.MethodsCollected 200 Ab isolates from January 2014 to December 2014.Antibiotic susceptibility were identified by MIC,then β-lactamase genes including TEM,SHV,GES,IMP-1,VIM-1,AmpC,OXA-51,OXA-23,OXA-24 and OXA-58 were detected by PCR.ResultsThere were 116 multidrug resistant Ab in all isolates.The rate of drug resistance to common antibiotics of mutidrug-resistant Ab was more than 70%.Fifty-two isolates (26.0%) were positive for TEM,135 (67.0%) were positive for SHV,7 (3.5%) were positive for GES,170 (85%) were positive for VIM-1 while 12 (6.0%) for IMP-1,150 isolates (75.0%) were detected carrying AmpC,190 (95.0%) were positive for OXA-51,165 (82.5%) were positive for OXA-23,only 2 (1.0%) were positive for OXA-58.OXA-24 was not detected in any of the isolates.ConclusionCarrying β-lactamase genes may be one of the important mechanisms of drug resistance of Ab.OXA-51,OXA-23,AmpC,VIM-1 and SHV were the most frequent genes.The carrying rates of all these genes were over 60.0%.Results showed that the drug resistance may be the result of synergistic effect of multiple β-lactamase genes.
