摘要:
目的 探讨肺结核患者外周血CD4+T细胞的表达,并对相关因素进行Logistic回归评估。方法 对2014年3月~8月期间苏州市第五人民医院肺科住院病例141例,分肺结核患者组(95例,男性71例,女性24例,平均年龄48.47±20.34)和肺部非结核疾病患者组(46例,男性32例,女性14例,平均年龄44.04±18.31),确诊后采取外周血3 ml经EDTA-K2抗凝,采用流式细胞仪进行外周血CD4+T细胞亚群检测; 采用五分类血常规仪检测外周血白细胞总数、淋巴细胞比率,从而计算CD4+T细胞的绝对数。采用SPSS 19.0软件进行统计分析。连续性变量正态分布用均数±标准差((-overx)±s)表示,两组间比较用成组设计资料的t检验; 非正态分布采用中位数表示,两组间比较用非参数检验中的秩和检验; 计数资料两组率的比较用χ2检验,采用多因素的非条件Logistic回归分析,计算不同CD4+T细胞水平下发生肺结核危险的比值比。结果 ①肺结核组与肺部非结核疾病组CD4+T细胞绝对数分别为313.20(189.90,418.00),400.81(278.84,541.37),采用非参数检验中的秩和检验分析,两组间差异有统计学意义(P=0.003); ②肺结核患者多因素(性别、年龄)的非条件Logistic回归分析发现,CD4+T细胞绝对数与肺结核的关联性具有统计学意义(P=0.009),而淋巴细胞总数与肺结核无联性。③以CD4+T细胞最低四分位(肺结核CD4+T细胞<217.4×109)分四组进行非条件Logistic回归分析,结果显示发生肺结核的危险性随CD4+T细胞水平的增加而呈现下降趋势,每两组间差异具有统计学意义(P=0.034,0.010,0.003),且经多因素(性别、年龄)调整后这种差异依然存在(P=0.038,0.012,0.005)。结论 CD4+T细胞可作为评价肺结核的鉴别指标及其危险性趋势的辅助指标,有利于临床对肺结核病的发生发展进行免疫治疗及预防。
Abstract:
Objective To study the expression of CD4+T cells in peripheral blood of pulmonary tuberculosis patients and assess the related factors by Logistic regression mode.Methods 141 inpatients(March -August,2014)from pulmonary medicine department of the Fifth People's Hospital of Suzhou were divided into two groups.Group 1 contains 95 pulmonary tuberculosis patients(71cases for male and 24 cases for female,mean age:48.47±20.34)and group 2 indued 46 lung infection patients(32 cases for maleand 14 cases for female,mean age:44.04±18.31).Took 3 ml of EDTA anti-coagulated peripheral blood from every patient and determined the percentage of CD4+T cells in through flow cytometry.Using five classification blood test instrument,WBC and lymphocyte ratio in the peripheral blood was detected to calculate the absolute number of CD4+T cells.Used SPSS 19.0 software to make the statistical analysis.Normal distribution of continuous variables was expressed asmean±standard deviation and two groups were compared by T test for group designdata.Non-normal distribution was expressed by the median and two groups werecompared by rank sum test in non-parametric test.Compared the rate of two groups by χ2 test and calculated the odds ratio of the tuberculosis risk in different level of CD4+T cells from non-conditional logistic regression analysis of multiple factors.Results ①The count of CD4+T cells in patients with pulmonary tuberculosis was 313.20(189.90,418.00)whilein patients with lung infections was 400.81(278.84,541.37).Use of rank sum test in non-parametric test showed a significant difference in the count of CD4+T cells between pulmonary tuberculosis and lung infections patients(P=0.003).②Non-conditional logistic regression analysis of multiple factors(gender,age)in pulmonary tuberculosis patients showed that the count of CD4+T cells was associated with pulmonary tuberculosis infection(P=0.009),whilethe total number of lymphocytes wasn't associated.③Making CD4+T cells to the lowest quartile(TB CD4+T cells <217.4×109)as a reference,141 cases were divided into four groups.The results obtained from non-conditional logisticregression analysis showed that the risk of pulmonary tuberculosis infection decreased with the increased levels of CD4+T cells.The difference between the groups was statistically significant(P=0.034,0.010,0.003)and even after multiple factors(gender,age)were adjusted,the result remained same(P=0.038,0.012,0.005).Conclusion CD4+T cells can beused as an auxiliary indicator for both the identification and the susceptibility of tuberculosis.Detecting CD4+T cells not only contribute to the treatmentof tuberculosis but also prevent the occurrence of tuberculosis.
