摘要:
目的 探讨乙肝病毒血清标志物、外周血T淋巴细胞与HBV-DNA的相关性。方法 273例乙肝患者采用ELISA法测定乙肝血清标志物HBsAg,HBsAb,HBeAg,HBeAb和HBcAb,采用流式细胞术测定T淋巴细胞亚群CD3+,CD4+和CD8+,并采用荧光定量PCR法测定HBV-DNA,比较不同血清标志物模式间HBV-DNA阳性表达情况,并分析其相关性。结果 HBsAg(+)HBeAg(+)HBcAb(+)组的HBV-DNA阳性率最高,为88.9%,HBV-DNA高浓度组CD3+ CD4+细胞百分比及CD3+CD4+/CD3+ CD8+比值低于HBV-DNA阴性组(P<0.05),CD3+ CD8+细胞百分比高于HBV-DNA阴性组(P<0.05); HBV-DNA拷贝数与CD3+ CD8+细胞百分比及HBeAg(r2=0.550,0.657,P<0.01)呈正相关,与CD3+ CD4+细胞百分比(r2=-0.602,P<0.05),HBeAb(r2=-0.473,P<0.01),HBcAb(r2=-0.151,P<0.05)呈负相关。结论 HBV-DNA与T淋巴细胞及HBeAg检测相关性较好,联合检测乙肝血清标志物、外周血T淋巴细胞及HBV-DNA能提高乙肝的检出率,有助于全面监测乙肝病情及评价药物疗效。
Abstract:
Objective To investigate the correlations studyon serum markers of hepatitis B virus(HBV)and peripheral T lymphocytes as well as HBV-DNA in patients with hepatitis B.Methods For 273 patients with hepatitis B,ELISA assay was used to determine the levels of HBsAg,HBsAb,HBeAg,HBeAb and HBcAb,flow cytometry was applied to determine peripheral T lymphocytes,and real-time quantitative PCR was used to determine HBV-DNA,the peripheral T lymphocytes and HBV-DNA expression among different serum marker modes were compared.The correlations among different markers were analyzed.Results The mode of HBsAg(+)HBeAg(+)HBcAb(+)hadthe highest HBV-DNA positive rate of 88.9% among all the modes.The CD3+ CD4+ T cell percentage and CD3+ CD4+/CD3+ CD8+ ratio of high concentration of HBV-DNA were significantly lower than those of HBV-DNA negative group(P<0.05),and the CD3+ CD8+ T cell percentage of high concentration of HBV-DNA was statistically higher than that of HBV-DNA negative group(P<0.05); HBV-DNA copies was positively correlated with CD3+ CD8+ T cell percentage and HBeAg(r2=0.550 and 0.657,P<0.01),and was negatively correlated with CD3+ CD4+ T cell percentage(r2=-0.602,P<0.05),HBeAb(r2=-0.473,P<0.01)and HBcAb(r2=-0.151,P<0.05).Conclusion There was a well correlation among HBV-DNA T lymphocytes and HBeAg,combined detection of these serum markers,peripheral T lymphocytes and HBV-DNA can improve the detection rate of hepatitis B,and thus contributes to monitoring patients conditions and evaluating drug effects.
