摘要:
目的 检测原发性舍格伦综合征(pSS)患者外周血辅助性T细胞22(Th22)水平,探讨其临床意义。方法 选取2014年1月～2015年11月期间收治的37例pSS患者为研究对象,归为观察组,并按照1:1的比例选取同期来上海市第六人民医院南院奉贤区中心医院体检的37例健康成人为对照组。采用流式细胞术检测所有受试者外周血中Th22细胞水平,酶联免疫吸附法(ELISA)检测血浆IL-22蛋白水平。采用Pearson分析观察组中外周血中Th22细胞水平与血浆IL-22蛋白水平,C3,C4,抗SSA抗体、抗SSB抗体、抗ANA抗体、欧洲风湿病联盟制定的SS病情评估指数(ESSDAI)之间的关系,并比较不同唇腺病理分级患者的Th22细胞水平与血浆IL-22蛋白水平。结果 观察组的Th22细胞水平与血浆IL-22水平分别为(2.53±1.56)%,718.6±176.8 pg/ml,显著高于对照组(1.24±0.51)%,258.9±72.4 pg/ml,差异有统计学意义(t=4.781,P<0.001; t=14.636,P<0.001)。观察组患者的Th22细胞水平与血浆IL-22水平、抗SSA 抗体、抗SSB抗体、ESSDAI总分等呈显著正相关,与C3,C4含量呈显著负相关(P<0.05)。观察组中不同病理分级患者的Th22细胞水平、血浆IL-22水平比较,差异无统计学意义(F=1.124,P=0.354>0.05; F=1.332,P=0.281>0.05)。结论 pSS患者外周血Th22细胞水平与血浆IL-22水平显著升高,且其与其他炎症指标与病情有关,可能参与了pSS的发生发展。
Abstract:
Objective To investigate the change of Th22 cells in peripheral blood of patients with Primary sjogren's syndrome(pSS)and evaluate clinical significance.Methods 37 patients with pSS from January 2014 to November 2015 were enrolled the study as the observationgroup.Then 37 healthy adults receiving check-up during the same period were selected as control groups in accordance with the proportion of 1:1.Flow cytometry was performed to evaluate the levels of Th22 cells in peripheral blood,andtenzyme-linked immuno sorbent assay(ELISA)was used to measure the serum IL-22 levels.Pearson analysis was performed to investigate the relationship between Th22 level and serum IL-22 level,C3,C4,anti-SSA,anti-SSB,ANA antibody,EULAR Sjogren's syndrome disease activity index(ESSDAI)score in observationgroup.Then the levels of Th22 cells and serum IL-22 were compared among different labial gland pathologic stage in patients with pSS.Results The levels of Th22 cells and serum IL-22 in the observation group were(2.53±1.56)%,718.6±176.8 pg/ml respectively,and significantly higher than(1.24±0.51)%,258.9±72.4 pg/ml in control group(P<0.05).Th22 cell level was positively related with the level of serum IL-22,anti-SSA,anti-SSB and ESSDAI score,and negatively related with the content of C3 and C4(P<0.05).There were no significant differences in the levels of Th22 cells andserum IL-22 different labial gland pathologic stage in patients with pSS(P>0.05).Conclusion The levels of Th22 cells and serumIL-22 significantly increased in patients with pSS,and related to other inflammatory indexes and disease activity,so they may participate in the genesis anddevelopment of pSS.
