目的 回顾性分析中南大学湘雅医院2015年分离的鲍曼不动 杆菌的临床分布特点及耐药情况,并分析其对常用抗生素的十年耐药变迁,为指导临床合理 选用抗生素及控制感染提供理论指导。方法 收集2015年湘雅医院临 床分离鲍曼不动杆菌534株,按标本种类、科室、年龄分布及药敏结果分析鲍曼不动杆菌的 临床分布特点及耐药性;同时与2006年和2011年湘雅医院鲍曼不动杆菌耐药情况进行比较。 结果 呼吸道标本中鲍曼不动杆菌分离率为76.78%,61岁以上感染 患者分离率为37.83%,重症监护室、呼吸内科、神经内科及神经外科四个科室合 计分离率为57.87%。2015年10种抗生素中对鲍曼不动杆菌抗菌活性最好的是头胞哌酮/舒巴 坦,耐药率为3.02%;其余9种抗生素均有不同程度的耐药,耐药率50%~85%。十年间上升 幅度最大的是妥布霉素和亚胺培南,2006年耐药率分别为1.87%和11.70%,2015年分别上 升至69.85%和80.53%,左氧氟沙星和头孢吡肟十年间耐药率也增长较快,升高幅度分别为 58.81%和55.98%。结论 2015年湘雅医院临床分离鲍曼不动杆菌主 要来源于呼吸道标本,主要分布在ICU,感染人群主要是老年患者。近10年来鲍曼不动杆菌 对常用抗生素耐药率显著升高,尤其是头孢类和碳青霉烯类抗生素。随着泛耐药及多重耐药 菌株逐渐增多,应加强对鲍曼不动杆菌耐药性监测及合理选用抗菌药物。
Abstract:
Objective The clinical distribution and drug re sistance of isolated Acinetobacter Bauman strains were retrospectively analy zed in Xiangya Hospital Central South University in 2015, and its resistant tra nsitions in ten years were also analyzed,to provide theoretical guidance for cl inical rational use of antibiotics and control of infection.Methods 534 Acinetobacter Bauman strains were collected at Xiangya Hosp ital in 2015,the clinical distribution characteristics and drug resistance of Acinetobacter Bauman strains were analyzed according to specimen type,depar tment,age and drug resistance result.Meanwhile,the drug resistance situation was compared with that in 2006 and 2011.Results The isol ation rate of Acinetobacer Bauman from respiratory tract specimen was 76.78 %,the tolal isolation rate from ICU,respiratory department of internal medicin e,neurology department of internal medicine and neurosurgery department ward wa s 57.87%,and the infection patients over the age of 61 occupied 37.83%.Rispe ridone/Shubatan showed the best antibiotic activity with the resistance rate 3. 02% in ten drugs,the Acinetobacter Bauman strains were resist to other nine drugs in different level,with the percent from 50.0% to 85.0%.The drugs of tobramycin and imipenem showed the greatest growth,with the resistance rate of 1.87% and 11.70% in 2006 in Xiangya hospital,increased to 69.85% and 80.53% in 2015,respectively.The drug of levofloxacin and cefepime also showed great growth in ten years with the increase of 58.81% and 55.98%,respectively.〖WT 5”HZ〗Conclusion The clinical isolates of Acinetobacter Bauman strains mainly derived from respiratory tract specimen,and mainly distributed i n ICU,the most infection people were from the older.The resistance rate of com mon drugs showed a significant increase in ten years,especially for cephalospor ins and carbapenems.With the increase of multiple drug resistant strains,the m onitoring of drug resistance of Acinetobacter Bauman and use of antibiotics rationally should be strengthened.
