摘要:
目的 探讨化学发光免疫分析法对丙型病毒性肝炎(丙肝)筛查的适用性,同时了解宁夏地区丙肝的感染现状,为其预防和控制提供科学依据。方法 收集2011~2015年在宁夏医科大学总医院就诊的采用化学发光免疫分析法(CIA)检测的所有丙肝抗体(抗-HCV)数据共209 889例,依据美国《2003CDC实验室抗-HCV检测及结果报告指南》,将有反应性病例分为弱阳性(0.9≤S/CO值≤8.0)和阳性(S/CO值>8.0)两组; 依据2015更新版《丙型肝炎防治指南》的临床诊断标准,完成急、慢性丙肝的确诊,采用卡方检验和回归分析的曲线估计等分析丙肝确诊率及患丙肝人群的分布特征。结果 S/CO值>8.0组,丙肝确诊率达97.57%; 0.9≤S/CO值≤8.0组,丙肝确诊率仅为2.05%。感染丙肝的人群,男女感染差异无统计学意义(χ2=1.432,P>0.05),21~60岁年龄段感染率高,儿童及老年人感染率较低(χ2=254.901,P<0.01),院内各科室均有分布; 假阳性人群主要为老年、儿童、孕妇及肿瘤患者,主要科室分布为产科、肿瘤科、儿科及口腔颌面外科。结论 化学发光免疫分析法检测抗-HCV适于丙肝临床筛查,但要注意其假阳性。应提高宁夏地区高发人群丙肝的检测,并注意院内感染的防治,不断完善对丙肝的筛查和监测体系。
Abstract:
Abstract:Objective The purpose of this study is to explore the applicability of chemiluminescent immunoassay in screening hepatitis Cinfection,and to find the infection status of heptitis C virus in Ningxia,and to provide scientific basis for its prevention and control.Methods A total of 209 889 hepatitis C antibody(anti-HCV)data detected by chemiluminescent immunoassay(CIA)were collected from the General Hospital of Ningxia Medical University during 2011~2015.According to the 《Laboratory Hepatitis C Antibody Detection and Results Report Guide of 2003 CDC》of the United States,Cases were divided into weak positive(0.9≤S/CO value≤8.0)and positive(S/CO value>8.0 )two groups.According to the clinical diagnostic criteria of 《Hepatitis C Prevention Guide》 of 2015 version to complete the diagnosis of acute and chronic hepatitis C.The chi square test and curve estimation ofregression analysis was used to analyze the diagnosis rate of hepatitis C and the distribution characteristics of the patients with hepatitis C.Results In the S/CO value>8.0 group,the diagnosis rate of hepatitis Cwas 97.57%.In the 0.9≤S/CO value≤8.0 group,the diagnosis rate of hepatitis C was only 2.05%.Among the people infected with HCV,there was no significant difference between male and female(χ2=1.432,P>0.05).The infectionrate was high in the individuals of ages between 21~60 years old,the infection rate of children and the elderly was low(χ2=254.901,P<0.01),almost all Departments of the hospital were distributed.The majority of false positive groups were elderly,children,pregnant women and tumor patients.The maindepartments were obstetrics,oncology,pediatrics and oral and maxillofacial surgery.Conclusion The method of Chemiluminescent immunoassay to detect anti-HCV is suitable for clinical screening of hepatitis C,butit is necessary to pay attention to the treatment of false positives.It is necessary to improve the detection of hepatitis C in the population with high prevalence in Ningxia,and pay attention to the prevention and treatment of nosoco-mial infection,and constantly improve the screening and monitoring system for hepatitis C.
