摘要:
目的 检测抗中性粒细胞胞浆抗体相关性血管炎(AAV)患者血清中类风湿因子(RF)的表达,并讨论RF与2003版伯明翰血管炎活动性评分(BVAS2003)量表的相关性。方法 收集华中科技大学同济医学院附属同济医院2017年3～9月确诊的76例AAV患者,全部研究对象于空腹抽取静脉血,应用酶联免疫吸附试验测定血清中IgM-RF,IgG-RF和IgA-RF的水平,同时采用免疫比浊法分析C反应蛋白(CRP)的浓度,利用BVAS2003量表对所有AAV患者进行评分。结果 36.8%的AAV患者血清中IgG-RF为阳性,且IgG-RF阳性患者CRP浓度较IgG-RF阴性患者明显升高,差异具有统计学意义(P<0.05),血清IgG-RF水平与BVAS2003分值呈正相关(r=0.26,P<0.05)。结论 AAV患者血清IgG-RF滴度与疾病活动度具有相关性,提示IgG-RF水平可用于AAV疾病的活动度评估。
Abstract:
Abstract:Objective To detect the serum level of rheumatoid factor(RF)in patients with antineutrophil cytoplasmic antibody-associatedvasculitis(AAV)and discuss the correlation with 2003 Birmingham Vasculitis Activity Score(BVAS2003).Methods From March 2017 to September 2017,76 cases of AAV diagnosed in the Affiliated Tongji Medical Hospital of Tongji Medical College of Huazhong University of Science and Technology.All subjects were collected for venous blood on fasting.Enzyme linked immunosorbent assays were used to measure the level of RFs.C-reactive protein(CRP)wasanalyzed by immunoturbidimetric assays.BVAS2003 was used to evaluate the activity of AAV patients.Results There are about 36.8% of AAVpatients with IgG-RF positive,and the level of CRP was higher in the RF-positive patients compared to the RF-negative subset(P<0.05).In addition,IgG-RF titers were significantly correlated with the BVAS2003(r=0.26,P<0.05).Conclusion In patients with AAV,IgG-RF titers weresignificantly correlated with disease active,which indicated IgG-RF may be function as a potential disease activity index of AAV.
