摘要:
目的 检测血液神经元特异性烯醇化酶(NSE),高敏C-反应蛋白(hsCRP)和中性粒细胞淋巴细胞比率(NLR)在不同严重程度的创伤性颅脑损伤患者中的水平变化并探讨其相关价值。方法 将117例创伤性颅脑损伤患者按GCS评分分为轻、中、重三组,并选择40例健康体检者作为对照组,分别检测患者和健康对照组NSE,hsCRP和NLR水平,并分析三个指标相关性。结果 与对照组相比,创伤性颅脑损伤(traumatic brain injury,TBI)患者血液NSE(t=13.66,P<0.01),hsCRP(t=17.00,P<0.01)和NLR(t=6.43,P<0.01)均显著升高,且随损伤程度的加重,三项指标升高程度增加。进一步分组结果显示,TBI患者NSE,hsCRP和NLR水平在轻型组分别为21.33±6.08 μg/L,15.31±3.74 mg/L和2.19(1.36~4.32),在中型组为32.86±12.67 μg/L,35.35±13.19 mg/L和1.92(0.86~2.47),重型组为58.09±26.25 μg/L,60.908 3±31.70 mg/L和6.49(4.61~9.39),与对照组中三者的水平8.71±5.54 μg/L,2.73±1.59 mg/L和1.745(1.188~2.95)相比,除NLR在轻型组升高水平不显著(P=0.188)外,其他组别和检测指标均显著升高(P<0.01)。在TBI患者中,轻、中、重三组NSE和hsCRP血清水平均存在明显差异(P<0.01),NLR水平在轻型组与重型组,中型组与重型组中均存在明显差异(P<0.05)。相关性分析显示,TBI患者NSE与NLR显著相关(r=0.440,P<0.01),NSE与hsCRP显著相关(r=0.571,P<0.01),NLR与hsCRP显著相关(r=0.416,P<0.01)。结论 TBI患者血液NSE,hsCRP水平和NLR指标明显升高且具有相关性,三项指标联合检测有助于TBI的临床诊断和分型。
Abstract:
Objective To investigate the levels of NSE,hsCRP and NLR in traumatic brain injury(TBI)patients and evaluate their correlation with the disease severity.Methods The levels of NSE,hsCRP and NLR were determined in patients with slight,medium and severe TBI according to Glasgow coma scale(GCS)score and healthy control group.The correlation of them was explored.Results Compared with control group,the levels of NSE,hsCRP and NLR in TBI group were significantly different(t=13.66,17.00,6.43,all P<0.01).The three parameters showed 21.33±6.08 μg/L,15.31±3.74 mg/L and 2.19(1.36~4.32)in slight TBI patients,32.86±12.67 μg/L,35.35±13.19 mg/L and 1.92(0.86~2.47),in medium TBI patients,and 58.09±26.25 μg/L,60.908 3±31.70 mg/L and 6.49(4.61~9.39)in heavy patients respectively.Compared with control group,the three indexes were all significantly elevated,except for the level of NLR in slight TBI group.NSE levels were positively correlated with NLR and hsCRP(r=0.440,0.571,P<0.01),and NLR were positively correlated with hsCRP(r=0.416,P<0.01).Conlusion There were statistical significance of NSE and hsCRP in three different severity TBI groups.Combined detection of three parameters is helpful for clinical diagnosis and typing of TBI.
