目的 分析急性脑梗死(acute cerebral infarction,ACI)患者进行血清超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、基质金属蛋白酶抑制剂-1(tissue inhibitor of metalloproteinase 1,TIMP-1)水平检测在疾病预后评估中的应用价值。方法 将2017年3月～2018年1月西安交通大学医学部附属3201医院收治的103例急性脑梗死患者设为病例组,根据临床神经功能缺损程度分级(NIHSS)将其分为轻度组46例,中度组42例,重度组15例,分别在入院第1,3,7,14,28天检测其血清hs-CRP,TNF-α和TIMP-1水平。另选同期105例健康体检者为正常对照组进行上述血清学指标检测。分析急性脑梗死患者的血清hs-CRP,TNF-α和TIMP-1水平变化与其预后的关系。结果 ①病例组患者在入院第1,3,7,14,28天的血清hs-CRP,TNF-α和TIMP-1水平均显著高于正常对照组,差异有统计学意义(t=15.390～33.167,P<0.05)。且在病例组中,随NIHSS分级程度的增加,患者血清hs-CRP,TNF-α和TIMP-1水平均会随之升高,差异具有统计学意义(P<0.05)。②治疗28天后,以病例组患者预后康复情况为依据将其分为基本治愈、显效、好转、无变化四组。其中,基本治愈组、显效组、好转组患者的血清hs-CRP,TNF-α,TIMP-1水平显著低于无变化组患者,差异具有统计学意义(F=24.080～39.623,均P<0.01)。经Spearman相关性分析发现,急性脑梗死患者血清hs-CRP,TNF-α和TIMP-1水平与其预后均呈负相关(P<0.05)。结论 在急性脑梗死患者病情发生、发展过程中其血清hs-CRP,TNF-α和TIMP-1水平会出现动态的变化,可将其用于患者病情变化以及预后评估中,临床应用价值较高。
Abstract:
Objective To analyze the applied significance of detection of serum hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and tissue inhibitor of metalloproteinase 1(TIMP-1)ofacute cerebral infarction(ACI)in the disease prognosis assessment.Methods 103 patients with acute cerebral infarction who were admitted to 3201 Hospital Affiliated College of Xi'an Jiaotong Univerity from March2017 to January 2018,were set as the case group.They were divided into 46 cases in the mild group,42 cases in the moderate group and 15 cases in the severe group by the clinical neurologic defect degree grading(NIHSS).The patients weredetected the levels of serum hs-CRP,TNF-α and TIMP-1 on the 1,3,7,14 and 28 day after admission respectively.In addition,105 healthy patients duringthe same period were selected as the normal control group for the above serological indicators test.Results ①Serum hs-CRP,TNF-α andTIMP-1 levels of patients in case group were significantly higher than those in the normal control group on the 1,3,7,14 and 28 day after admission(t=15.390～33.167,P<0.05).In the case group,with the increase of NIHSS grading,the levels of Serum hs-CRP,TNF-α and TIMP-1 also increased.The differences were statistically significant(P<0.05).②After 28-day treatment,the patients were divided into four groups according to their prognosis and rehabilitation,namely,basic cure group,obvious effect group,improved group and no-changing group.The levels of serum hs-CRP,TNF-α and TIMP-1 in the basic cure group,the obvious effect group and the improved group were significantly lower than those in the on-changing group.The differences were statistically significant(F=24.080～39.623,P<0.01).Spearman correlation analysis showed thatserum hs-CRP,TNF-α and TIMP-1 levels of acute cerebral infarction patientswere negatively correlated with their prognosis(P<0.05).Conclusion The serum hs-CRP,TNF-α and TIMP-1 levels of patients withacute cerebral infarction would change dynamically in the course of disease occurrence and development.It can be used to determine the patient's condition changes and prognosis assessment.It also has high clinical application value.
