摘要:
目的 探讨胃蛋白酶原(PGⅠ,PGⅡ)及相关肿瘤标志物与胃癌的相关性。方法 采用全自动生化分析仪,罗氏电化学发光仪检测胃癌组患者(192例)、胃炎组患者(135例)及健康对照组人群(164例)血清中PGⅠ,PGⅡ,糖类抗原(CA-724,CA-199)和癌胚抗原(CEA)水平,计算PGⅠ/PGⅡ(Ⅰ/Ⅱ)值,分析上述指标与胃癌的相关性。结果 ①胃癌组、胃炎组和健康对照组间PGⅠ,PGⅡ,Ⅰ/Ⅱ,CA-724,CA-199和CEA差异均有统计学意义(F=27.56,36.54,27.87,19.78,16.89,19.01,均P<0.05); ②胃癌组中转移组PGⅠ,Ⅰ/Ⅱ,CA-199水平高于未转移组,差异有统计学意义(t=29.36,6.79,6.33,均P<0.05); ③I/II与胃癌TNM分期呈负相关(r=-0.513,P=0.007 1),CA-724,CA-199与胃癌TNM分期呈正相关(r=0.452,0.427,P=0.008 2,0.042); ④六种血清肿瘤标记物在胃癌中的敏感度、特异度和准确度不同,其中以Ⅰ/Ⅱ敏感度(68.38%)、特异度(97.10%)、准确度(65.39%)较高。结论 Ⅰ/Ⅱ,CA-724,CA19-9与胃癌具有较强的相关性,并且敏感度、特异度较高; 在胃癌转移监测中PGⅠ/PGⅡ比值,CA-199更具有临床价值。
Abstract:
Objective Objectived to explore the relationship between pepsinogen(PGⅠ,PGⅡ)and other tumor markers and gastric cancer.Methods The PGⅠ,PGⅡ,CA-724,CA-199 and CEA levels inserum of patients with the gastric cancer group(192 cases),the gastritis group(135 cases)and the healthy control group(164 cases)were detected by automatic biochemical analyzer and Roche electrochemical luminescence method.CalculatedPGⅠ/PGⅡ(Ⅰ/Ⅱ)value and analysed their relationship with gastric cancer.Results ①There were significant differences in PGⅠ,PGⅡ,Ⅰ/Ⅱ,CA-724,CA-199 and CEA between the three groups in the gastric cancer group,the gastritis group and the healthy control group(F=27.56,36.54,27.87,19.78,16.89,19.01,all P<0.05).②The levels of PGⅠ,Ⅰ/Ⅱ and CA-199 in the gastric cancer group were higher than those in the non metastasis group(t=29.36,6.79,6.33,all P<0.05).③Ⅰ/Ⅱ was negatively correlated with TNM staging of gastric cancer(r=-0.513,P=0.007 1)and CA-724,CA-199 was positively correlated with TNM staging of gastric cancer(r=0.452,0.427,P=0.008 2,0.042).④The sensitivity,specificity and accuracy of six serum tumor markers in gastric cancer were different.Among them,Ⅰ/Ⅱ sensitivity(68.38%),specificity(97.10%),accuracy(65.39%)was highest.Conclusion Ⅰ/Ⅱ,CA-724 and CA19-9 had strong correlation with gastric cancer,and the sensitivity and specificity were high.In the monitoring of metastasis of gastric cancer,PGⅠ,PGⅠ/PGⅡ and CA-199 had more clinical value.
