摘要:
目的 探讨慢性心力衰竭(chronic heart failure,CHF)患者呋塞米治疗对血清N端前脑钠肽原(NT-proBNP)的影响。方法 收集渭南市第一医院2012年2月~2017年4月收治的180例慢性心力衰竭患者,按照就诊号排序后取随机数,前90例为观察组,后90例为对照组,两组均给予常规的药物治疗,观察组在常规药物治疗基础上给予不同剂量呋塞米注射液(基础剂量组、中剂量组及高剂量组,每组30例)。基础剂量组:呋塞米40~80 mg/d滴注; 中剂量组:呋塞米80~120 mg/d滴注; 高剂量组:呋塞米160 mg/d滴注,两组患者在治疗前、治疗3,7天采用免疫荧光法检测NT-proBNP水平,比较不同时间两组患者NT-proBNP水平及分析不同剂量呋塞米与NT-proBNP变化的相关性。结果 两组患者均随着治疗时间延长NT-proBNP水平逐渐降低,差异具有统计学意义(F=12.341,34.218,均P<0.05); 治疗前NT-proBNP水平组间差异无统计学意义(t=0.563,P>0.05),治疗后观察组在治疗3天、7天NT-proBNP水平均低于对照组同期水平,差异具有统计学意义(t=5.783,6.874,均P<0.05); 观察组亚组NT-proBNP水平在治疗3天、7天组间差异均具有统计学意义(F=16.894~30.673,均P<0.05),其中中剂量组和高剂量组NT-proBNP水平均低于基础剂量组,高剂量组低于中剂量组,差异具有统计学意义(t=8.897,9.453,均P<0.05); 不同剂量呋塞米与NT-proBNP水平变化间呈现正相关关系(rs=0.673,P<0.05)。结论 呋塞米可以有效地降低慢性心力衰竭患者血清NT-proBNP水平,并且剂量越大,效果越明显。
Abstract:
Objective To investigate the effect of furosemide on serum N in patients with chronic heart failure(CHF)of terminal brain natriuretic peptide(NT-proBNP)effect.Methods 180 cases of CHF patients in the First Hospital of Weinan from February 2012 to April 2017.According to the random number and the number of visits after sorting,the first 90 cases of the observation group,90 cases in the control group,two groupswere given routine drug treatment,and the observation group were given different doses of furosemide injection on the basis of the conventional treatment(basal dose group,middle dose group and high dose group,30 cases in each group).based dose group:40~80 mg/d furosemide infusion; middle dose group:80~120 mg/d furosemide infusion; high dose group:160 mg/d furosemide infusion,patients in the two groups before and after treatment for 3 and 7 days by immunofluorescencedetection of NT-proBNP levels between the two groups in different time NT-proBNP and correlation analysis of different doses furosemide and NT-proBNP changes.Results Two groups of patients with prolonged treatmentNT-proBNP levels decreased,the difference was statistically significant(F=12.341~34.218,P<0.05).The difference of NT-proBNP level between groups was not statistically significant(t=0.563,P>0.05)before treatment.Aftertreatment in the observation group after treatment,3d and 7d NT-proBNP were lower than the control group during the same period,the difference was statistically significant(t=5.783,6.874,P<0.05)NT-proBNP level of the observation group.Subgroup was statistically significant difference in treatment 3 d,7d group(F=16.894~30.673,all P<0.05),the middle dose group and high dose group were lower than NT-proBNP based dose group,high dose group was lower than that of the middle dose group,the difference was statistically significant(t=8.897,9.453,all P<0.05).Showed a positive correlation between the changes of different doses of furosemide and NT-proBNP level(rs=0.673,P<0.05).Conclusion Furosemide can effectivelyreduce the serum NT-proBNP level in patients with CHF, and the higher the dose,the effect is more obvious.
