摘要:
目的 评价血清5种生物标志物水平对心力衰竭(HF)患者的诊断价值。方法 采用化学发光法与定量ELISA改进法检测不同程度HF患者的血清5种生物标志物水平,运用IBM SPSS23.0与SAS 9.4软件对结果进行统计处理。结果 Ⅰ~Ⅳ级HF患者的血清5种生物标志物水平明显增高,N末端脑钠肽(NT-proBNP)、中段心房利钠肽原(MR-proANP)、大内皮素1(BigET-1)、和肽素(copeptin)与半乳糖凝集素-3(Gal-3)的结果分别为3.09±0.37,3.53±0.55,3.70±0.52和3.93±0.46 ng/L,2.50±0.27,3.01±0.60,3.21±0.58和3.43±0.63pmol/L,3.97±1.34,5.43±1.80,6.14±1.80和7.09±1.98 pmol/L,16.12±3.19,19.22±3.74,21.30±3.86和23.54±4.42 pmol/L,29.94±4.06,34.35±5.68,36.58±5.90和39.60±6.35 μg/L,差异均具有统计学意义(F=71.804128.045.P<0.000)。5种生物标志物的实验受试者特征曲线下面积分别为0.988 5,0.986 6,0.949 2,0.978 3和0.977 4,它们两两比较,9对有差别,1对无差别。用NT-proBNP≥719.5 ng/L,MR-proANP≥192.5 pmol/L,Big ET-1≥3.45 pmol/L,和肽素≥14.0 pmol/L与Gal-3≥27.15 μg/L来联合诊断HF患者,有2项符合即可诊断。敏感度为98.6%,特异度为98.2%,诊断符合率为98.5%。结论 联合检测血清5种生物标志物,能提高HF诊断的敏感度和特异度。
Abstract:
Objective To evaluate the diagnostic value of serum 5 biomarkers in patients with heart failure(HF).Methods The serum levels of 5 biomarkers in HF patients with different degrees were detected by chemiluminescence and modified quantitative enzyme-linked immunosorbent assay.IBM SPSS23.0 and SAS9.4 software were used for statistical processing of the results.Results The serum levels of 5 biomarkers were significantly elevated in patients with class Ⅰ~Ⅳ HF.The resultsof N-terminal pro-brain natriuretic peptide,midregional pro-atrial natriuretic peptide,big endothelin-1,copeptin and galectin-3 were 3.09±0.37,3.53±0.55,3.70±0.52 and 3.93±0.46 ng/L; 2.50±0.27,3.01±0.60,3.21±0.58 and 3.43±0.63 pmol/L; 3.97±1.34,5.43±1.80,6.14±1.80 and 7.09±1.98 pmol/L; 16.12±3.19,19.22±3.74,21.30±3.86 and 23.54±4.42 pmol/L,29.94±4.06,34.35±5.68,36.58±5.90 and 39.60±6.35 μg/L,respectively.The differences were statistically significant(all P=0.000).Thearea under of receiver operator characteristic curve for 5 biomarkers were 0.988 5,0.986 6,0.949 2,0.978 3 and 0.977 4,respectively.When they were compared in pairs,nine pairs were different and one pair was no difference.HF patients were diagnosed with NT-proBNP≥719.5 ng/L,MR-proANP≥192.5 pmol/L,Big ET-1≥3.45 pmol/L,copeptin≥14.0 pmol/L and Gal-3≥27.15 μg/L,and two conformance could be diagnosed.The sensitivity,specificity and diagnostic coincidence rate were 98.6%,98.2% and 98.5% respectively with two results to diagnose HF.Conclusion Combined detection of 5 biomarkers in serum can improve the sensitivity and specificity of HF diagnosis.
