摘要:
目的 探究联合检测血清中缺血修饰蛋白(ischemic modified protein,IMA)、心型脂肪酸结合蛋白(cardiac fatty acid binding protein,H-FABP)与超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)水平对急性冠脉综合征(acute coronary syndrome,ACS)早期诊断的应用价值。方法 选取2017年9月~2018年12月就诊于吉林省人民医院ACS患者共63例, 其中23例不稳定型心绞痛(unstable angina pectoris,UAP)患者作为UAP组、21例非ST段抬高型心肌梗死(non-ST-elevation myocardial infarction,NSTEMI)患者作为NSTEMI组、19例ST段抬高型心肌梗死(ST-elevation myocardial infarction,STEMI)患者作为STEMI组。选取30例健康人群作为对照组。分别比较四组受试者IMA,H-FABP与hs-CRP三项指标的检测水平,同时计算并比较IMA,H-FABP与hs-CRP单独及联合检测的灵敏度及受试者工作曲线(receiver operating characteristic,ROC)的曲线下面积(area under curve,AUC)。结果 UAP组的IMA,H-FABP与hs-CRP检测水平分别为69.19±13.67 U/ml,18.66±15.47 ng/ml和1.94±1.81 mg/dl; NSTEMI组的IMA,H-FABP与hs-CRP检测水平分别为72.18±13.04 U/ml,25.68±19.62 ng/ml和3.93±3.32 mg/dl; STEMI组的IMA,H-FABP与hs-CRP检测水平分别为78.85±14.78 U/ml,78.44±52.69 ng/ml和12.62±11.03 mg/dl,结果显示实验组的检测水平明显高于对照组,差异均有统计学意义(F=23.48880.921,P<0.05)。三组中IMA,H-FABP和hs-CRP联合检测的灵敏度分别为83%,86%和89%,均优于各组中三项指标单独检测的灵敏度。三项指标联合检测的AUC值为0.915,较单独检测准确度更高。结论 采用IMA,H-FABP与hs-CRP联合检测的方法可显著提高ACS早期诊断的准确度,为临床早期ACS诊断提供更为可靠有效的检测依据。
Abstract:
Objective To investigate the value of combineddetection of serum ischemia-modifying protein(IMA),cardiac fatty acid binding protein(H-FABP)and high-sensitivity C-reactive protein(hs-CRP)levelsin the early diagnosis of acute coronary syndrome(ACS).Methods A total of 63 patients with ACS who were admitted to the People's Hospital of Jilin Province from September,2017 to December,2018 were enrolled.Among them,23 patients with unstable angina pectoris(UAP)were UAP group,21 patients with non-ST-elevation myocardial infarction(NSTEMI)were NSTEMI group,19patients with ST-elevation myocardial infarction(STEMI)were STEMI group.30healthy people were selected as the control group.The detection levels of IMA,H-FABP and hs-CRP were compared among the four groups.Calculate and compare the sensitivity of IMA,H-FABP and hs-CRP alone and in combination,and the area under curve(AUC)of the receiver operating characteristic curve(ROC curve).Results The levels of IMA,H-FABP and hs-CRP in the UAPgroup were 69.19±13.67 U/ml,18.66±15.47 ng/ml and 1.94±1.81 mg/dl,respectively.The levels of IMA,H-FABP and hs-CRP in the NSTEMI group were 72.18±13.04 U/ml,25.68±19.62 ng/ml and 3.93±3.32 mg/dl,respectively.Thelevels of IMA,H-FABP and hs-CRP in the STEMI group were 78.85±14.78 U/ml,78.44±52.69 ng/ml and 12.62±11.03 mg/dl,respectively.And the results were significantly higher than those in the control group(F=23.488~80.921,all P<0.05).The sensitivity of combined detection of IMA,H-FABP and hs-CRP in the experimental group was 83%,86% and 89%,respectively,and the sensitivity of each group was better than that of three indicators alone.The AUC of the combined detection of the three indicators was 0.915,and the diagnostic accuracy was higher.Conclusion The combination of IMA,H-FABP and hs-CRP can significantly improve the accuracy of early diagnosis of ACS,and provide a more reliable and effective basisfor early diagnosis of ACS.
